Tesla, Inc. (Tesla) is recalling certain 2012-2024 Model S, 2015-2024 Model X, 2017-2023 Model 3, and 2020-2023 Model Y vehicles. In the event of an unbelted driver, the seat belt warning light and audible chime may not activate as intended. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 208, "Occupant Crash Protection."

Potential Risk:
A seat belt warning system that fails to alert occupants of an unbelted seat belt can increase the risk of injury during a crash.

Safety Issue:

Tesla, Inc. (Tesla) is recalling certain 2012-2023 Model S, 2016-2024 Model X, 2017-2023 Model 3, 2019-2024 Model Y, and 2024 Cybertruck vehicles. An incorrect font size is displayed on the instrument panel for the Brake, Park, and Antilock Brake System (ABS) warning lights. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 105, "Hydraulic and Electric Brake Systems" and 135, "Light Vehicle Brake Systems."

Potential Risk:
Warning lights with a smaller font size can make critical safety information on the instrument panel difficult to read, increasing the risk of a crash.

(Tesla) is recalling 2012-2023 Model S, 2016-2023 Model X, 2017-2023 Model 3, and 2020-2023 Model Y vehicles equipped with all versions of Autosteer leading up to the version(s) that contains the recall remedy. In certain circumstances when Autosteer is engaged, the prominence and scope of the feature's controls may not be sufficient to prevent driver misuse of the SAE Level 2 advanced driver-assistance feature.

Potential Risk:
In certain circumstances when Autosteer is engaged, and the driver does not maintain responsibility for vehicle operation and is unprepared to intervene as necessary or fails to recognize when Autosteer is canceled or not engaged, there may be an increased risk of a crash.

Safety Issue:

Tesla, Inc. (Tesla) is recalling certain 2016-2023 Model S, Model X, 2017-2023 Model 3, and 2020-2023 Model Y vehicles equipped with Full Self-Driving Beta (FSD Beta) software or pending installation. The FSD Beta system may allow the vehicle to act unsafe around intersections, such as traveling straight through an intersection while in a turn-only lane, entering a stop sign-controlled intersection without coming to a complete stop, or proceeding into an intersection during a steady yellow traffic signal without due caution. In addition, the system may respond insufficiently to changes in posted speed limits or not adequately account for the driver's adjustment of the vehicle's speed to exceed posted speed limits.

Potential Risk:
FSD Beta software that allows a vehicle to exceed speed limits or travel through intersections in an unlawful or unpredictable manner increases the risk of a crash.

The rearview image may not immediately display when the vehicle begins to reverse. Tesla, Inc. (Tesla) is recalling certain 2012-2018 Tesla Model S and 2016-2018 Model X vehicles with a center display equipped with a NVIDIA Tegra 3 processor and an 8GB eMMC NAND flash memory device. When the 8GB eMMC NAND flash memory device for the center display reaches lifetime wear, the eMMC controller will no longer be able to maintain the integrity of the filesystem, causing a failure in some of the center display functions.

Potential Risk:
The eMMC controller wear-out condition can cause the loss of the rearview camera display, defrost/defog control settings, and exterior turn signal lighting, reducing visibility and increasing the risk of a crash.
FREE Recall Solution:
Owners should ensure their vehicles are operating firmware release 2020.48.48.12 or newer, which will alert owners if the eMMC is approaching lifetime wear. Tesla will notify owners, and will replace the VCM daughterboard with one containing an enhanced eMMC controller, free of charge. The recall began March 29, 2021. FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING

Potential Consequences:

Starting especially so on this date, and on occasion before this time, The vehicle would abruptly brake very aggressively when I was passing a semi truck with his emergency lights on or when perhaps passing a shadow or for no reason at all. I would have to slam on the gas to avoid a collision or being rear ended.

Defect Description:

SUSPENSION

Potential Consequences:

The rear suspension has design problem that even within the alignment spec and normal driving conditions, the inner side of the rear tires are wearing off in an astonishing speed. This is s a known issue to all customers. Just in my personal case, the inner side of the rear tire wear off to thread within about 5000 miles driving and began leaking. This has extremely serious potential danger in local and highway driving situations. Since the normal car maintenance work hardly could take it nner side tire data. Very high percentage of drivers are driving the car with danger now on the road without any knowledge and preparness for the danger. This put not only the Tesla MX driver in danger, but also everyone around the vehicle, which leads this to a imminent public safety issue. Also, the manufacturer's scheduled maintenance is way inadequate, especially they deny any liability on n this problem, and never officially remind the consumer until the problem surfaced. This policy is also extremely unfair to Tesla's frontline workers. They have experience hostile environments from real world to execute this dangerous company policy. A immediate mandatory recall is highly suggested to inspect all MX models affected. Before a sound technical solution is perfected, consumers should be advised with options to safely drive this car, or this car should be banned from public roads.

About 224 miles N. of Los Angeles, on I-5, Southbound, the car aggressively braked while using traffic aware cruise control. Speed was around 75 when braking started. Speed dropped to 55 MPH at the end. There was a couple additional brakes on this southbound journey. I can't believe that Tesla thinks they can implement full self driving when they can't manage to maintain a constant and not jarring speed. This is a recipe for disaster.

Corrective Action:

Tesla has owners make service appointment in an App which I have done multiple times in the past few years but each time the alert has gone away and the service appointments have been canceled. Turns out now, 4 years later, we have been driving with a faulty restraint system and there is a disconnect between the system and the wiring. Essentially this is a safety issue in that the notification system is faulty and incorrectly and only intermittently warns of possible failure as well as a possibility of the restraint system actually failing in a driving scenario. This problem is particularly concerning because they are unable to correctly diagnose which seats are affected and that includes seats that may have car seats attached.

While the vehicle was parked in my garage, after I removed a car seat from the rear driver side passenger seat, the door would not lock and stay closed. Because of this the vehicle would stay on and continue to give me a drivers side rear passenger door open alert. When I attempted to drive the vehicle, it would constantly give me a warning that the door was open and a distractive noise alert would activate that hindered or disturbed my ability to concentrate on the road. I went to the tesla Pomona service center and they insisted that it was safe to drive and road worthy. While on the freeway I could hear air come into the vehicle and I could also smell diesel exhaust. Whenever I parked the vehicle, the vehicle would remain on and unlocked, which would drain the battery.

The latching mechanism on the larger of the two pieces failed in a way that left the seat free to slide backwards and forwards. This is extremely dangerous as it could result in crush injuries to the occupant of the seat. If a child seat was installed, it could result in fatal whiplash. I have a video of the seat moving freely as pictures don't really help describe the problem.

Corrective Action:

Under normal acceration..the car shake and vibrate violently. This is a known Tesla design flaw on all MODEL X. Tesla even issue a Technical Service Bulletin to address the design flaw. This shudder...shake... and vibration is very dangerous and could cause the drive shaft to separate from the drive unit. FORWARD COLLISION AVOIDANCE: WARNINGS

Potential Consequences:

While driving on an interstate freeway on 8-14-2021, using the vehicle Autopilot driver assistance mode (which I had been using for the previous 18 months without incident), the vehicle failed to avoid hitting the vehicle in front of me. This collision resulted in no passenger injury, but resulted in both driver and passenger airbags to engage and resulted in total vehicle loss of the Tesla Model X. No other vehicles involved had airbag engagement. Tesla may have actual information on the functioning of the vehicle and the Autopilot software that may explain why the collision avoidance system failed, but refused to supply me with this information.
