Tesla, Inc. (Tesla) is recalling certain 2012-2024 Model S, 2015-2024 Model X, 2017-2023 Model 3, and 2020-2023 Model Y vehicles. In the event of an unbelted driver, the seat belt warning light and audible chime may not activate as intended. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 208, "Occupant Crash Protection."

Potential Risk:
A seat belt warning system that fails to alert occupants of an unbelted seat belt can increase the risk of injury during a crash.

Safety Issue:

Tesla, Inc. (Tesla) is recalling certain 2012-2023 Model S, 2016-2024 Model X, 2017-2023 Model 3, 2019-2024 Model Y, and 2024 Cybertruck vehicles. An incorrect font size is displayed on the instrument panel for the Brake, Park, and Antilock Brake System (ABS) warning lights. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 105, "Hydraulic and Electric Brake Systems" and 135, "Light Vehicle Brake Systems."

Potential Risk:
Warning lights with a smaller font size can make critical safety information on the instrument panel difficult to read, increasing the risk of a crash.

Tesla, Inc. (Tesla) is recalling 2012-2023 Model S, 2016-2023 Model X, 2017-2023 Model 3, and 2020-2023 Model Y vehicles equipped with all versions of Autosteer leading up to the version(s) that contains the recall remedy. In certain circumstances when Autosteer is engaged, the prominence and scope of the feature's controls may not be sufficient to prevent driver misuse of the SAE Level 2 advanced driver-assistance feature.

Potential Risk:
In certain circumstances when Autosteer is engaged, and the driver does not maintain responsibility for vehicle operation and is unprepared to intervene as necessary or fails to recognize when Autosteer is canceled or not engaged, there may be an increased risk of a crash.

Safety Issue:

Tesla, Inc. (Tesla) is recalling certain 2016-2023 Model S, Model X, 2017-2023 Model 3, and 2020-2023 Model Y vehicles equipped with Full Self-Driving Beta (FSD Beta) software or pending installation. The FSD Beta system may allow the vehicle to act unsafe around intersections, such as traveling straight through an intersection while in a turn-only lane, entering a stop sign-controlled intersection without coming to a complete stop, or proceeding into an intersection during a steady yellow traffic signal without due caution. In addition, the system may respond insufficiently to changes in posted speed limits or not adequately account for the driver's adjustment of the vehicle's speed to exceed posted speed limits.

Potential Risk:
FSD Beta software that allows a vehicle to exceed speed limits or travel through intersections in an unlawful or unpredictable manner increases the risk of a crash.

The "rolling stop" functionality available as part of the Full Self-Driving (Beta) software may allow the vehicle to travel through an all-way stop intersection without first coming to a stop.

Potential Risk:
Failing to stop at a stop sign can increase the risk of a crash.

Tesla, Inc. (Tesla) is recalling certain 2014-2021 Model S vehicles. The front trunk latch assembly may be misaligned, preventing the secondary hood latch from engaging. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 113, "Hood Latch System."

Potential Risk:
If the primary latch is inadvertently released and the secondary latch is not engaged, the hood could open unexpectedly, obstructing the driver's view and increasing the risk of a crash.

Tesla, Inc. (Tesla) is recalling certain 2012-2018 Tesla Model S and 2016-2018 Model X vehicles with a center display equipped with a NVIDIA Tegra 3 processor and an 8GB eMMC NAND flash memory device. When the 8GB eMMC NAND flash memory device for the center display reaches lifetime wear, the eMMC controller will no longer be able to maintain the integrity of the filesystem, causing a failure in some of the center display functions.

Potential Risk:
The eMMC controller wear-out condition can cause the loss of the rearview camera display, defrost/defog control settings, and exterior turn signal lighting, reducing visibility and increasing the risk of a crash.
FREE Recall Solution:
Owners should ensure their vehicles are operating firmware release 2020.48.48.12 or newer, which will alert owners if the eMMC is approaching lifetime wear. Tesla will notify owners, and will replace the VCM daughterboard with one containing an enhanced eMMC controller, free of charge. The recall began March 29, 2021. My vehicle driver door handle keeps popping open while parked and while driving. The door handle will either present itself or Not when this occurs. Disallowing me keep door shut. Also, door stays shut as times and doesn’t not present itself and makes a mechanical sound. This is a HUGE safety concern. I was advised since my warranty is up, it’s on me to fix, however, this is a safety defect and I am wanting to escalate ASAP before I have an accident from the door opening.

Unknown My daytime driving lights for safety have begun to go out. It has been a long standing complaint but they refuse to fix mine under an expired warranty and I was quoted $1650 each to replace. I was told they would do a good will replacement but they will not. There are many complaints on the Tesla forums.

The backup camera stopped working. I took the vehicle to the Tesla dealership and had to have the camera replaced. I am aware that there is a recall on Model 3 Tesla’s for a camera issue. I have also seen multiple sites online where Tesla drivers report issues with the backup camera for Model S vehicles.

Potential Consequences:

On March 22, 2021 Tesla announced a recall of the 8GB MultimediaCard (eMMC) for my 2017 Tesla Model S. It causes the main touchscreen to go fail, rear camera to fail, lose of control of radio/defrost and all other commands that require this touch screen. Tesla said they would replace the eMMC. That was almost a year ago. My vehicle has experienced this failure multiple times both before and after the notice and yet still nothing. I actually took my car in for this failure in Nov 2020 and was told nothing was wrong. Its an intermittent failure which requires a reboot to fix temporarily.

Corrective Action:

The left front fore link assembly broke while I was driving approximately 30mph. See photo. The car made a bad vibration noise when braking or driving over a bump. I was barely able to drive it back home. The car did not have braking ability. I had to have it towed to the nearest Tesla Service Center (80 miles from my home). If this failure had happened at high speed, I imagine this could have resulted in an accident. I had the car repaired, and Tesla had a replacement part different from the original part, and also replaced the right side. It seems they are very aware of this part’s failure. Between the towing and repair, the total cost was approximately $1725.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL

Potential Consequences:

Additional report for same problem with DIFFERENT car. This report is not about a single incident, but rather a REPEATED problem over several years with the adaptive cruise control on 3 different Tesla cars I have owned. These cars are a 2017 Tesla S, and 2 different 2021 Tesla Y models. in each case, whenever these cars are driven on a highway with the adaptive cruise control engaged, several times during each hour of driving, the cars will rapidly brake/decelerate with no action taken to cause the situation by the driver, nor any external situations with the traffic that would justify them. With the Tesla regenerative braking, these deceleration events are so extreme that it would be easy to imagine an accident with a car following closely behind. Indeed, with these situations occurring with no warning and extreme braking they are quite frightening to the passengers traveling in the car. During the most recent occurrence, I noted the exact day and minute of it's occurrence, and filed a service request with the local Tesla office. Even with the exact time of this event, the service center was unable to find anything wrong with the car. The Tesla service center attributed this issue to problems with the Autopilot and Full Self Driving software, but none of my cars have had this optional software-only the software which came standard on each of these cars. While otherwise appreciating the good characteristics of these cars, this unexplainable deceleration problem is what caused me to sell the prior 2 models before purchasing this latest model Y, hoping that this problem would not recur. It is worth noting that I have owned several other make and model cars with comparable adaptive cruise control, yet this situation never occurred even once on any of these other makes of cars.

Corrective Action:

Additional report for same problem with DIFFERENT car. This report is not about a single incident, but rather a REPEATED problem over several years with the adaptive cruise control on 3 different Tesla cars I have owned. These cars are a 2017 Tesla S, and 2 different 2021 Tesla Y models. in each case, whenever these cars are driven on a highway with the adaptive cruise control engaged, several times during each hour of driving, the cars will rapidly brake/decelerate with no action taken to cause the situation by the driver, nor any external situations with the traffic that would justify them. With the Tesla regenerative braking, these deceleration events are so extreme that it would be easy to imagine an accident with a car following closely behind. Indeed, with these situations occurring with no warning and extreme braking they are quite frightening to the passengers traveling in the car. During the most recent occurrence, I noted the exact day and minute of it's occurrence, and filed a service request with the local Tesla office. Even with the exact time of this event, the service center was unable to find anything wrong with the car. The Tesla service center attributed this issue to problems with the Autopilot and Full Self Driving software, but none of my cars have had this optional software-only the software which came standard on each of these cars. While otherwise appreciating the good characteristics of these cars, this unexplainable deceleration problem is what caused me to sell the prior 2 models before purchasing this latest model Y, hoping that this problem would not recur. It is worth noting that I have owned several other make and model cars with comparable adaptive cruise control, yet this situation never occurred even once on any of these other makes of cars.

The contact owns a 2017 Tesla Model S. The contact stated while charging the vehicle at a charge station, the contact entered the vehicle and after a few minutes, an abnormal plastic burning odor was present in the interior of the vehicle. The contact noticed white smoke coming from the front of the hood of the vehicle. The contact exited the vehicle and unpluged the charger from the vehicle and waited a few minutes for the white smoke to clear from the front of the vehicle on its own. No warning lights illuminated the vehicle, and the only message that appeared on the vehicle monitor was charging was almost complete. The vehicle was towed to a local dealer who diagnosed the failure as the fan condenser had overheated and it would need to be replaced. The vehicle was repaired, however, the contact had not picked up the vehicle from the dealership at this time. The manufacturer was made aware of the failure. The failure mileage was approximately 23, 227.

Las Vegas,NV approximately 9:00AM an unknown motorist cut me off on surface streets. I had to apply brakes hard manually not in driving assist mode in an hard panic stop to avoid collision. When I resumed driving I heard crunching and scraping sounds coming from driver side front wheel. I pulled over and inspected an noticed tire was close to wheel well. I the drove carefully to nearby Tesla dealership. Problem was diagnosed as a fore link failure. It is important to note there was no impact whatsoever. I don’t think suspension components should fail after hard braking at approximately 40 mph.
