As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."

FORWARD COLLISION AVOIDANCE: WARNINGS
Potential Consequences:
My car slammed on the brakes on the highway this morning at 75 mph. A box truck was following me but thankfully he didn't hit me. I was in what Tesla calls Traffic Aware Cruise Control (TACC) at the time and quickly brought the car out of TACC and hit the accelerator to get me back up to speed so I didn't pose a danger to other drivers on the highway. I dont know exactly how much my car slowed, but it was enough to throw me forward in my seat. There were no vehicles in front of me at the time. This was a classic example of "phantom braking", which is talked about a lot in the Tesla community. This is my 2nd report of phantom braking to the NHTSA.

The contact owns a 2024 Tesla Model Y. The contact stated that while at an undisclosed speed, the vehicle had accelerated independently. No warning lights were illuminated. On another occasion, while exiting a parking lot the vehicle unintendedly accelerated causing it to crash into a parked vehicle. The vehicle came to a stop partially sideways after it drove over the parked vehicle. The fire department arrived on the scene and safely removed the contact from the vehicle. It was unknown if a police or fire department report was filed. The contact sustained minor bruises on the right arm and sought medical attention a day later. The vehicle was towed to an insurance lot where it remained for further inspection. The vehicle was not repaired. The manufacturer was made aware of the failure. The failure mileage was unknown.

Defect Description:
VISIBILITY/WIPER
Potential Consequences:
The auto wipers come on multiple times a drive without water being on the windshield. This has been dangerous because the wipers will smear material across the windshield at the wrong times creating a visibility hazard. It almost caused a wreck where it activated smearing the windshield while the sun was directly ahead. Tesla’s response was the auto wipers are in Beta mode and I should turn them off to keep them from randomly coming on. This is a challenge since you have to use the center screen to find the wipers and activate them causing one to look away from the roadway.
Corrective Action:

This happen once before earlier this year on a two lane highway when a truck was coming toward me in the opposite lane. Possible others. •How was your safety or the safety of others put at risk? oThe seat melted stuck to a gym (drawstring) bag I had placed on it. When removed, the hot, sticky parts of the seat stuck to my bag and hands. I was put at risk for potential burns. oA person could get stuck on the seat potentially putting them at risk when not being able to exit the vehicle if needed in a safety situation. oThe heat controls could be shorted in the seat potentially putting anyone near at risk of a heat or possible fire incident. •Has the problem been reproduced or confirmed by a dealer or independent service center? oNot that I am aware of. The contact stated that only the passenger's side air bags were deployed. The fire department and Police department arrived on the scene. Potential Consequences:
I was driving on a street at the speed limit there were no cars in front of me and behind me thankfully the car just braked very hard suddenly for no reason and I lost complete control over the vehicle after the car came to a complete stop I was able to continue driving. Thankfully there were no cars behind me because if there were I don’t even want to imagine the accident I could’ve caused aside from this the breaking was so hard that my whole body hurts and I am afraid to drive the vehicle as I do not know when to expect this breaking since it can happen so suddenly at any time.
