As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."

I noticed that now Tesla uses windshield mounted cameras for everything in front of car including FSD and forward collision. My model y was made in fremont, California. Problem is windshield glass is not cleaned before assembly to car. Either not a prescribed step by Tesla during assembly or overlooked by assembly workers. I noticed after I picked up my new model y from Tesla that there is haze and layer of residue on windshield glass. As if glass are sitting in bulk in factory and assembled to car without cleaning. I was able to clean windshield inside with glass cleaner and microfiber cloth. Problem is windshield haze and Layer of residue cannot come out of front facing camera module. This is causing all functions to degrade that rely on these windshield mounted cameras. I get random "one of cameras occulted" error during Full self driving FSD. Car cuts out during summon. Haze can be seen on app cameras during summon, while rear camera and fender cameras show clear view. This will get worse with winter now as fog also sets on windshield. FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
Potential Consequences:
While driving between 50-55 mph the Tesla applied emergency breaking even though there was nothing in front of us and no risk of accident. The car went from 55 to about 5-10mph in the matter of a second. We are lucky no one was directly behind us or it would have led to a high speed rear end accident.

What component or system failed or malfunctioned, and is it available for inspection upon request? The "Actually Smart Summon" failed. It is available for inspection. How was your safety or the safety of others put at risk? The "Actually Smart Summon" function did not detect the post next to my vehicle as it was exiting the parking space. It immediately turn right into the post instead of pulling straight out first. I had very little reaction time. If there had been a person in the position of the post, this could have caused physical harm. My vehicle sustained damage to the front passenger door, its side mirror, and to the PPF material over these components. Has the problem been reproduced or confirmed by a dealer or independent service center? I don't know Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? No Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? In the past few weeks, I have had to completely replace both rear OEM aluminum alloy wheels due to cracks caused by normal road driving, which caused both rear tires to go flat. Per Tesla, this costly replacement is not covered under their warranty. I am now learning that this is a common issue with these particular wheels on this particular model (Model Y Performance). It appears the aluminum alloy wheels are simply not sturdy enough to bear the weight of the vehicle and the required tires simply do not provide sufficient cushioning to prevent wheel cracks resulting from normal wear and tear/road driving. Many customers have had to replace the OEM wheels with aftermarket wheels made of forged steel. These customers report zero issues with the much stronger wheels. This appears to be a product design or manufacturing problem, which is creating a significant consumer/public health risk.

On a couple occasions there was traffic behind me, and it must have scared them. They probably thought it was a hard brake check. I am personally afraid to use it now, knowing it’s going to slam on the brakes is nerve wracking and UNSAFE. From reading online forums it seems to affect just about everyone. Rather than turn right, the car continued straight. Potential Consequences:
My car slammed on the brakes on the highway this morning at 75 mph. A box truck was following me but thankfully he didn't hit me. I was in what Tesla calls Traffic Aware Cruise Control (TACC) at the time and quickly brought the car out of TACC and hit the accelerator to get me back up to speed so I didn't pose a danger to other drivers on the highway. I dont know exactly how much my car slowed, but it was enough to throw me forward in my seat. There were no vehicles in front of me at the time. This was a classic example of "phantom braking", which is talked about a lot in the Tesla community. This is my 2nd report of phantom braking to the NHTSA.
