The FSD Beta system may allow the vehicle to act unsafe around intersections, such as traveling straight through an intersection while in a turn-only lane, entering a stop sign-controlled intersection without coming to a complete stop, or proceeding into an intersection during a steady yellow traffic signal without due caution. In addition, the system may respond insufficiently to changes in posted speed limits or not adequately account for the driver's adjustment of the vehicle's speed to exceed posted speed limits.

FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
Potential Consequences:
The contact owns a 2023 Tesla Model Y. The contact stated while driving 70 MPH with the Adaptive Cruise Control or Autonomous Self Driving feature activated, the vehicle experienced phantom braking. No warning light was illuminated. The vehicle was not diagnosed nor repaired by an independent mechanic or dealer. The manufacturer was not made aware of the failure. The failure mileage was approximately 1,008.
Corrective Action:

Potential Consequences:
What component or system failed or malfunctioned, and is it available for inspection upon request? — Driver’s side rear view mirror. Mirror vibrates to a point where rear view is blurred and no longer clearly visible to driver. How was your safety or the safety of others put at risk? No event, however I have been unable to clearly see traffic behind me when traveling 60mph+ due this defect. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes, I have taken it to the Tesla dealership in Raleigh, NC 3 times for the same issue. They have openly acknowledged the defect across all Model Y’s and have no fix. The team tightened the mirror, replaced the mirror, added padding behind without any luck. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Yes, I drove the vehicle with a Tesla mechanic at the Tesla dealer in Raleigh, NC— he acknowledged the defect. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? This is a Day 0 manufacturing issue since I purchased the vehicle.

FORWARD COLLISION AVOIDANCE: WARNINGS
Potential Consequences:
I am filing the following two incidents regarding false and faulty warnings of the forward collision system. I also filed "bug reports" with the manufacturer, will ask for a service appointment. 1.On June 7, 2023 at 9:10am while I was driving from Fairfax, VA to Arlington, VA on Rte 50, this section of the road had 3 lanes, the left most lane was blocked with a car signaling for a left turn. I am in the middle lane, which is slowed down, there is plenty of distance between my car and the one in front of me. At that point of time, I received a false alarm. 2. On June 9, 2023 at 8:11pm while I was traveling with family on Rte 620, one lane slowed down due to approaching a roundabout my car fired another faulty forward collision warning. Everybody in the car is confused, trying to figure out what is going on. There is plenty of distance, double the legally required, between my card and the car in front of me.

I drive in West Texas, no hills but some rises and slopes, mostly flat. Going from Odessa to Lubbock (5AM) maybe 2 phantom braking episodes. Coming back on a clear, sunshiny day I experienced 15-20, out of the blue. Scared me to death. No cars within 100 yards, just BAM and it braked. Another 6-7 when the car thought a car over taking me was a problem and braked or one coming in a two lane (just about 5 miles only) thinking there was a danger. We are talking about excellent 4 lane highways with excellent pavement and markings.

Potential Consequences:
The adaptive cruise control was on today at and around 10:30am on highway 101N in the Bay Area, CA from Woodside exit on 101N to 6th street exit on 280 in SF. The listed highway speed started changing erratically causing the car to break to slow without cause and I had to take over to avoid being rear ended. There were no cars ahead of me that were close enough to cause breaking, and no construction zone or signage either. It was the listed highway speed that kept changing erratically from 65mph to as follows: 25 mph for a few minutes in a 65mph zone, then back to 65mph 25mph for a few minutes in a 65mph zone, then back to 65mph 35mph for maybe a minute in a 65mph zone, then back to 65mph 55mph for a short minute in a 65mph zone, then back to 65mph 30mph for a minute or so in a 65mph zone, then back to 65mph 25mph for a few minutes in a 65mph zone

The braking reduced speed by at least 30-40 mph in a matter of a second or two. This occurred 6 times during this drive. While using Autopilots Traffic-Aware Cruise Control and the Autosteer feature the car will without warning begin braking. Each time is different in regards to severity of the breaking but it is typically like letting off the accelerator pedal which in a Tesla is comparable to significant breaking. Have taken two highway road trips and on both trips, the phantom breaking has occurred multiple times. Some of the incidents the car would go from 75MPH to 45MPH in a matter of seconds. From research done, this "Phantom Breaking" is a serious issue that Tesla says is due to software issues and not hardware issues. Our safety was put at risk because the person behind us might not be paying attention when cruising on open road, during daylight, etc and could rear-end our car.
