Tesla, Inc. (Tesla) is recalling certain 2024 Cybertruck, 2017-2025 Model 3, and 2020-2025 Model Y vehicles. The tire pressure monitoring system (TPMS) warning light may not remain illuminated between drive cycles, failing to warn the driver of low tire pressure. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 138, "Tire Pressure Monitoring Systems."

Potential Risk:
Driving with improperly inflated tires increases the risk of a crash.

A factory reset muted the Pedestrian Warning System (PWS) sounds. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 141, "Minimum Sound Requirements for Hybrid and Electric Vehicles."

Pedestrians may be unaware of an approaching vehicle if the PWS sounds are muted, increasing the risk of a crash.

In certain circumstances when Autosteer is engaged, the prominence and scope of the feature's controls may not be sufficient to prevent driver misuse of the SAE Level 2 advanced driver-assistance feature.

In certain circumstances when Autosteer is engaged, and the driver does not maintain responsibility for vehicle operation and is unprepared to intervene as necessary or fails to recognize when Autosteer is canceled or not engaged, there may be an increased risk of a crash.

Safety Issue:

Tesla, Inc. (Tesla) is recalling certain 2016-2023 Model S, Model X, 2017-2023 Model 3, and 2020-2023 Model Y vehicles equipped with Full Self-Driving Beta (FSD Beta) software or pending installation. The FSD Beta system may allow the vehicle to act unsafe around intersections, such as traveling straight through an intersection while in a turn-only lane, entering a stop sign-controlled intersection without coming to a complete stop, or proceeding into an intersection during a steady yellow traffic signal without due caution. In addition, the system may respond insufficiently to changes in posted speed limits or not adequately account for the driver's adjustment of the vehicle's speed to exceed posted speed limits.

Potential Risk:
FSD Beta software that allows a vehicle to exceed speed limits or travel through intersections in an unlawful or unpredictable manner increases the risk of a crash.

Potential Risk:
Swapped camera views can prevent the vehicle's cameras from detecting crossing objects and high-curvature lane lines, impairing the Autosteer and Automatic Emergency Braking (AEB) performance, and increasing the risk of a crash.

Potential Risk:
Contact with the rim may cause a loss of tire pressure, increasing the risk of a crash.
FREE Recall Solution:
Tesla Service will inspect and tighten, or replace the caliper bolts as necessary, free of charge. Owner notification letters were mailed July 23, 2021. FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL

Potential Consequences:

While adaptive cruise control is enabled, the car will hard brake at random. The screen doesn't indicate why the car is randomly braking, not are there any other indicators as why or when this will happen. This occurs most often while going around corners, up hills, or when traffic is oncoming in the other lane. This happens on standard roads and highways - although highways are a little better as long as the opposite bound lane is separated by a wide median. Contacted Tesla support December 16, 2021 in which their reply was that "the car is learning... apply constant pressure to pedal in order to override braking system". I did try that for a couple of months, and instead of hard braking, it is a little gentler, but then I run the risk of going over the speed limit as constant pressure has to be applied ontop of whatever the cruise control is set to. Not too mention this defeats the purpose of having cruise control.

This unanticipated event causes the car to immediately drop in speed between 5 to 30 mph. Any motorists following will not be prepared for this illogical behavior and significantly increases the probability of a rear end collision. This behavior of the Adaptive Cruise Control has also occurred when the car approaches oncoming highway traffic regardless of whether the highway is divided or not. In this event, if the driver of the Tesla model Y does not instantly take over complete control of the vehicle a severe rear end collision can occur. I have not addressed this with Tesla nor have I taken my car in for inspection. I have been told by an owner of a Model 3 and an owner of a Model S that they have also experienced similar situations caused by the poorly calibrated performance of the Tesla Adaptive Cruise Control system. I have been forced to stop using the Adaptive Cruise Control and Auto-steer on my Model Y as this problem is on going.

While using the adaptive cruise control on the highway, the car abruptly slowed down from approximately 70 mph to less than 55 within a few seconds before I was able to push the gas petal and reaccelerate. This has happened 3 other times in the last several weeks and I am now afraid to use the cruise control. Whereas no accident occurred, if it happened when another car was following me closely, it could have easily resulted in a crash. Each time this has happened, there were no other cars nearby so I don’t know what the car ‘saw’ and was reacting to. The date entered below for the incident is the most recent occurrence.

Potential Consequences:

When car was in "traffic aware cruise control" on open highway it has multiple times unexpectedly braked hard when there were no vehicles or objects in front of it. Once there was a car behind me so I had to accelerate hard to avoid being rear ended. Once it may have been reacting to an oncoming truck in the opposite lane.

My wife and I were driving south on Interstate-93 in NH on Superbowl Sunday. Traffic was heavy and we were traveling about 65 mph when our 2021 Model Y Tesla started beeping and the following message came on the screen - “PULL OVER SAFELY, Vehicle shutting down”. My wife was driving and had just enough time to cross over three lanes to get into the breakdown lane before the car came to a sudden stop. The car did not want to restart. We called the Tesla service center and they were about to order a tow truck but we asked if there was a ‘ctrl/alt/del’ type option. She had us select Safety>Power Off. When our Tesla came back on everything was seemingly normal so the service center said ‘this is very rare’ and that it should be okay to drive. We drove another few miles down 93 and luckily took an early exit ramp because the car shut down again at the bottom of the ramp. My wife and I restarted the car again and managed to limp home in some sort of “Power reduced” mode (i.e. painfully little acceleration). This was potentially a very dangerous situation! My Tesla Model Y has terrible Phantom Braking problems. The AutoPilot features on the Tesla is extremely dangerous. Phantom Braking can happened anytime whenever AutoPilot feature is used, it just suddenly slow the vehicle without warning, there’s no other vehicle in front of me. The problem occurred more on a less busy freeway/ highway, sometimes it’s totally useless because the the feature is too dangerous. On a two lane highway, when more car detect a larger vehicle on the opposite lane, it started slowing down, sometimes that’s very dangerous because cars behind me almost hit my car. Tesla’s AutoPilot is extremely dangerous, they should recall the car and fix the problem.
