Potential Risk:
A torn air bag may not adequately protect an occupant in a crash, increasing the risk of injury.

An incorrect font size is displayed on the instrument panel for the Brake, Park, and Antilock Brake System (ABS) warning lights. In certain circumstances when Autosteer is engaged, the prominence and scope of the feature's controls may not be sufficient to prevent driver misuse of the SAE Level 2 advanced driver-assistance feature.

In certain circumstances when Autosteer is engaged, and the driver does not maintain responsibility for vehicle operation and is unprepared to intervene as necessary or fails to recognize when Autosteer is canceled or not engaged, there may be an increased risk of a crash.

(Tesla) is recalling certain 2016-2023 Model S, Model X, 2017-2023 Model 3, and 2020-2023 Model Y vehicles equipped with Full Self-Driving Beta (FSD Beta) software or pending installation. The FSD Beta system may allow the vehicle to act unsafe around intersections, such as traveling straight through an intersection while in a turn-only lane, entering a stop sign-controlled intersection without coming to a complete stop, or proceeding into an intersection during a steady yellow traffic signal without due caution. In addition, the system may respond insufficiently to changes in posted speed limits or not adequately account for the driver's adjustment of the vehicle's speed to exceed posted speed limits.

Potential Risk:
FSD Beta software that allows a vehicle to exceed speed limits or travel through intersections in an unlawful or unpredictable manner increases the risk of a crash.

The Boombox function allows sounds to be played through an external speaker while the vehicle is in motion, which may obscure the Pedestrian Warning System (PWS) sounds. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 141, "Minimum Sound Requirements for Hybrid and Electric Vehicles."

Potential Risk:
Pedestrians may be unaware of an approaching vehicle if the PWS sounds are obscured, increasing the risk of a crash.

A software error may cause a valve in the heat pump to open unintentionally and trap the refrigerant inside the evaporator, resulting in decreased defrosting performance. Potential Consequences:

2022 Model X plaid is exhibiting “ Phantom Braking” while in autopilot mode using navigation. Vehicle abruptly brakes while driving on highway as navigation appears to become confused and thinks it is on a side street. Vehicle goes from high speeds of 70-80 mph, down to 30-40 mph. If was being followed closely, car behind Tesla would have collided with rear of vehicle. This has occurred 10+ times since purchased vehicle in February. Vehicle has just under 6000 mi. as of today. I have times/dates to include a video of one incident. I have brought it to Tesla for service on 2 separate occasions, to no avail. They say it needs the latest upgrade, which has not resolved issue.

Service advisor at local Tesla Svc center told me there are several other Tesla cars on the service lot with similar issues with high voltage battery packs and are all awaiting replacement packs.

This is regarding the 2022 Tesla Model X steering wheel/yoke The button for the left and right turning signal occasionally does not work. When I tried pressing down the bottom, it is stuck and does not produce the haptic vibration It at least malfunctions once every 2-3 days. This is especially dangerous when I am merging onto the freeway, when it suddenly stops working. Tesla first address the issue by replacing the yoke/steering wheel when I brought the car into service on 9/12/2022. However, shortly after leaving the service center, the issue shows up again. I then had to submit a new service request, which was supposed to get fix/replace on 10/14/2022. However, the service center said the part is not available yet, so pushed back the appointment date to 11/07/2022 But they cannot guarantee that the part will be available on that date as well. At this point, I am driving a car that post safety risk and put others at risk, as I am not able to use the turning signal 100%. There was a few times I almost got into accidents. I mention this to Tesla service support, but they brush it off and simply said part is not available There is no urgency to fix it. I have at least 3 videos of showing the turning signal not working while I am driving. During those times, luckily I had a passenger with me that can help record. Trying to secure a backless child safety booster seat which has latch tether devices. The Tesla model X 2022 3rd row does not have the latch tether bars. I believe the brand of seat is GoFit Chicco. My 7 year old granddaughter was only restrained with the shoulder lap belt. During a forced rapid deceleration emergency stop she was choked by the seatbelt when the booster seat slid off the seat during rapid deceleration. She was not injured other than a red mark, sorry no photos, and was significantly traumatized from the incident. This booster seat has a clear warning stating the latch system is required and does not have a built in seatbelt channel since it’s backless. The 2nd row seats were in use with two other child safety seats using the latch system which provided ample protection.

There is an issue with the GPS in that the location is not accurate, sometimes it is off by 10-20 feet, enough to put it on a different road, sometimes it's off by miles. This is very dangerous as when using Autopilot, it uses map data to set the cruising speed, and if the GPS is reporting the car is on a different road, the speed can change to being faster or slower than the actual road the car is on.

The 2021-2022 model X “yoke” steering wheel is poorly conceived and a safety hazard. 1) horn location is tiny, and moves as wheel turns, impossible to quickly activate horn in emergency situations 2) high Beam lights are on a tiny button which also moves and cannot be activated when wheel turns or in an emergency situation, or even when choosing to cross an intersection during a yellow light because a car behind you is tailgating. 3) wheel shape: when used at lower speeds in autopilot Beta mode, the electronics literally rip the wheel out of your hands when turning right on residential streets. Potential Consequences:

While driving, our tire lost pressure resulting in the tread and side wall of the tire to separate. This failure that occurred is the same defect reported on the Continental CrossContact LX Sport recalls. It was a huge safety issue as we were driving on interstate and had to safely get to other side and pull over. Tire size - 265/45 R20

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL

Potential Consequences:

"phantom braking" of the tesla optical-only (no radar or lidar) adaptive cruise control. northbound on hwy 101 around Gonzales CA there is a truck stop on the west side of the highway. proper signage and lane lines enable 101 traffic to maintain 65 mph. the insufficient optical-only system hit the brakes through the intersection and caused traffic behind me to brake suddenly as if there were an obstruction. i scheduled service but this is a system that should be tested much more rigorously before delivery. the sudden braking that the system employed caused a situation where driver input was required to avoid a collision. this is inherently dangerous and is avoidable by peers' ACC systems. has not been reproduced to my knowledge. no inspection of the system. yes, there is an audible alarm that the system is braking. too little, too late. one final thought: the ACC systems of vehicles in this class use radar and/or lidar to enable sampling of the entire electromagnetic spectrum that humans can monitor cost-effectively. for some reason tesla made the insane decison to go optical-only, robbing their system of at least 10 times the data. the detriment in available data that an optical-only system has to overcome is too great to solve with nifty software. tesla's decision endangers drivers of their cars and, most importantly, others on the road.

1) low-speed turning is very difficult. makes hitting pedestrians/curbs/edifices much more likely, as well as just holding up traffic and frustrating other drivers. 2) turn signals require looking down at the steering yoke unless you're a real ninja. even if you get the hang of them without looking down, it's too easy to make a mistake. also i've noticed that when i'm trying to use the left signal, the terrible haptic faceplate will sometimes activate the right turn signal. when this happens in traffic at speed on the highway it can cause accidents. 3) horn is a haptic region that you have to look for to activate. i think this is being addressed but it's not sufficient to recall just to replace the current version with one with a normal horn in the middle. the entire thing needs to be replaced with a wheel and turn signal stalk. Elon Musk might be a genius but he has no business running an ongoing car company.
