As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) numbers 208, "Occupant Crash Protection" and 212, "Windshield Mounting."

Potential Risk:
A torn air bag may not adequately protect an occupant in a crash, increasing the risk of injury.

An incorrect font size is displayed on the instrument panel for the Brake, Park, and Antilock Brake System (ABS) warning lights. In certain circumstances when Autosteer is engaged, the prominence and scope of the feature's controls may not be sufficient to prevent driver misuse of the SAE Level 2 advanced driver-assistance feature.

In certain circumstances when Autosteer is engaged, and the driver does not maintain responsibility for vehicle operation and is unprepared to intervene as necessary or fails to recognize when Autosteer is canceled or not engaged, there may be an increased risk of a crash.

The FSD Beta system may allow the vehicle to act unsafe around intersections, such as traveling straight through an intersection while in a turn-only lane, entering a stop sign-controlled intersection without coming to a complete stop, or proceeding into an intersection during a steady yellow traffic signal without due caution. In addition, the system may respond insufficiently to changes in posted speed limits or not adequately account for the driver's adjustment of the vehicle's speed to exceed posted speed limits.

FSD Beta software that allows a vehicle to exceed speed limits or travel through intersections in an unlawful or unpredictable manner increases the risk of a crash.

The Boombox function allows sounds to be played through an external speaker while the vehicle is in motion, which may obscure the Pedestrian Warning System (PWS) sounds. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 141, "Minimum Sound Requirements for Hybrid and Electric Vehicles."

Potential Risk:
Pedestrians may be unaware of an approaching vehicle if the PWS sounds are obscured, increasing the risk of a crash.

A software error may cause a valve in the heat pump to open unintentionally and trap the refrigerant inside the evaporator, resulting in decreased defrosting performance. Potential Consequences:

At around 9:10 p.m. on 7/26/2023, I was driving 2 guests on the I-5 highway from Seattle to the airport. The car’s automatic driving assistance system suddenly failed to work. Tip 1: The steering assistance effect has been reduced 2: Lane departure avoidance function is not available; At this time, the steering wheel suddenly couldn't turn, and I felt locked tightly, unable to change lanes, so I had to slowly reduce the speed, turn very, very hard to change lanes, from the innermost carpool lane to the outermost exit lane, slowly The driver controlled the steering wheel hard, and it was impossible to drive on the deceleration road when getting off the high speed. I was driving my Tesla model X in interstate I35 with FSD turned on. After 45 minutes of FSD, I felt the car slowed down. I immediately push the accelerator to speed it up but it didn't help and the car quickly fully stopped. I again pushed accelerator but the car won't move at all and my car block one of the two lanes of north bound I35 in Gainesville area. I was forced to reboot the car as the car won't move. Several minutes later, the car finally rebooted successfully and I immediately drove to exit I35. As I had no idea why the car stopped itself so I didn't feel to continue driving it and had the car towed back. From the trip data on my phone, it shows "hard breaking" prior to all this abnormal happened. I am sure that no break panel was pushed as my foot is on the floor mat as the car was driven by the FSD. The most serious problem is I couldn't speed it up after the car "hard breaking" itself and I had to reboot the car in order to make the car moving. I have the car service by Tesla but they don't have any explanation for me yet. It will be difficult to reproduce the problem. It can only be found by Tesla by looking into the stored driving data.

In numerous incidents, I have attempted to signal other drivers using my horn in an emergency situation. In one such incident, I was in the left lane, traveling at approx. 40 miles per hour when a driver who was stopped in the right lane began switching into my lane. I went to honk to indicate my presence, but the capacitive touch button on the upper right of the wheel was difficult to locate in a quick emergency situation. Additionally, pressing the center of the wheel, as will honk in every other car on the market did not work. In another incident, I was traveling through an intersection at approx. 50 miles per hour, and an oncoming vehicle attempted to make a left turn as they did not see me (presumably). I attempted to honk, but again, the horn button was very difficult to location in such a quick, high-stakes emergency situation. It is my opinion that Tesla's current design for their Model X and Model S vehicles, where the horn can only be initiated via a small capacitive touch button, represents a safety hazard to owners of these vehicles, as well as the general public who may be interacting with these vehicles on the road. Tesla has prioritized design over safety, and it is only a matter of time before lives are lost through easily preventable collisions due to the lack of a standard warning device such as a horn.

As I was trying to make a turn into the parking lot of a convenient store, my Tesla Model X started accelerating suddenly and unintendedly without my foot being on the accelerator. I kept pressing on the brake to stop the car, but it would not stop. I had no control and while it accelerated for 15-20 yards eventually it hit two other cars that were parked there. This happened so quickly. We all were terrified and shocked as I could not stop it from accelerating. I kept yelling "it is not stopping" to the person seating on the passenger side. I am confident that I did NOT press the accelerator pedal. The car was not on autopilot. My initial speed was probably 5 miles/hour, but it accelerated so suddenly that four airbags deployed, and the car is not drivable. Insurance company is evaluating whether the car is repairable, but I am afraid of driving with this car again until the problem is found and fixed. This could have turned to a major tragedy.

That's what happened earlier on 7/7 as well as in the parking lot. I scrolled to go forward, but it didn't scroll so when I hit the "gas" (its electric), it went backwards into the car in back of me. What if the car behind me was a person instead - that would have been awful. Tesla should fix this. 2) Tesla's horn is a small button on the right side of the 1/2 steering wheel. You can't even reach it with your right fingers when you are driving. If your right hand is not available, it is almost impossible to use your left finger to push exactly that small button for the horn. I also complained to Tesla about this safety issue when I had another problem with the car. Previously, someone cut me off and I couldn't even warn them because my right finger couldn't push exactly that small button.
