Safety Issue:

Tesla, Inc. (Tesla) is recalling certain 2012-2024 Model S, 2015-2024 Model X, 2017-2023 Model 3, and 2020-2023 Model Y vehicles. In the event of an unbelted driver, the seat belt warning light and audible chime may not activate as intended. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 208, "Occupant Crash Protection."

Potential Risk:
A seat belt warning system that fails to alert occupants of an unbelted seat belt can increase the risk of injury during a crash.

Safety Issue:

Tesla, Inc. (Tesla) is recalling certain 2012-2023 Model S, 2016-2024 Model X, 2017-2023 Model 3, 2019-2024 Model Y, and 2024 Cybertruck vehicles. An incorrect font size is displayed on the instrument panel for the Brake, Park, and Antilock Brake System (ABS) warning lights. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 105, "Hydraulic and Electric Brake Systems" and 135, "Light Vehicle Brake Systems."

Potential Risk:
Warning lights with a smaller font size can make critical safety information on the instrument panel difficult to read, increasing the risk of a crash.

(Tesla) is recalling 2012-2023 Model S, 2016-2023 Model X, 2017-2023 Model 3, and 2020-2023 Model Y vehicles equipped with all versions of Autosteer leading up to the version(s) that contains the recall remedy. In certain circumstances when Autosteer is engaged, the prominence and scope of the feature's controls may not be sufficient to prevent driver misuse of the SAE Level 2 advanced driver-assistance feature.

Potential Risk:
In certain circumstances when Autosteer is engaged, and the driver does not maintain responsibility for vehicle operation and is unprepared to intervene as necessary or fails to recognize when Autosteer is canceled or not engaged, there may be an increased risk of a crash.

Safety Issue:

Tesla, Inc. (Tesla) is recalling certain 2016-2023 Model S, Model X, 2017-2023 Model 3, and 2020-2023 Model Y vehicles equipped with Full Self-Driving Beta (FSD Beta) software or pending installation. The FSD Beta system may allow the vehicle to act unsafe around intersections, such as traveling straight through an intersection while in a turn-only lane, entering a stop sign-controlled intersection without coming to a complete stop, or proceeding into an intersection during a steady yellow traffic signal without due caution. In addition, the system may respond insufficiently to changes in posted speed limits or not adequately account for the driver's adjustment of the vehicle's speed to exceed posted speed limits.

Potential Risk:
FSD Beta software that allows a vehicle to exceed speed limits or travel through intersections in an unlawful or unpredictable manner increases the risk of a crash.

Tesla, Inc. (Tesla) is recalling certain 2012-2018 Tesla Model S and 2016-2018 Model X vehicles with a center display equipped with a NVIDIA Tegra 3 processor and an 8GB eMMC NAND flash memory device. When the 8GB eMMC NAND flash memory device for the center display reaches lifetime wear, the eMMC controller will no longer be able to maintain the integrity of the filesystem, causing a failure in some of the center display functions.

Potential Risk:
The eMMC controller wear-out condition can cause the loss of the rearview camera display, defrost/defog control settings, and exterior turn signal lighting, reducing visibility and increasing the risk of a crash.
FREE Recall Solution:
Owners should ensure their vehicles are operating firmware release 2020.48.48.12 or newer, which will alert owners if the eMMC is approaching lifetime wear. Tesla will notify owners, and will replace the VCM daughterboard with one containing an enhanced eMMC controller, free of charge. The recall began March 29, 2021. Tesla, Inc. (Tesla) is recalling certain 2016-2017 Model X vehicles. The left-side, second row, reclining seat backs may not fully latch due to having incorrectly adjusted recliner mechanism cables. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 207, "Seating Systems," and 210, "Seat Belt Assembly Anchorages."

Potential Risk:
If the seat back moves forward in the event of a crash, it can increase the risk of injury.

Tesla, Inc. (Tesla) is recalling certain 2016 Model S and Model X vehicles. The electric parking brake calipers have an internal gear that may be improperly manufactured, possibly resulting in the gear fracturing during parking brake application or release.

Potential Risk:
If the gear breaks during parking brake release, the vehicle will not be able to be moved. If the gear breaks during parking brake application, the parking brake may not adequately hold the vehicle, potentially resulting in the vehicle rolling, increasing the risk of a crash.
FREE Recall Solution:

Potential Consequences:

The contact owned a 2017 Tesla Model X. The contact's husband stated while driving 70 mph, a strange noise was heard coming from the rear driver side of the vehicle which resulted in the contact's husband losing control of the vehicle and collided with the median which brought the vehicle to a stop. None of the air bags deployed but no one was inquired and a police report was taken. The vehicle was towed to a junkyard where it was determined to be destroyed. The contact stated no warning lights was illuminated. The manufacturer was not informed of the failure. The failure mileage was unavailable.

The rear camber arms of both Tesla Model S and X suspension have fixed camber angle that only grows wider as the time passes, that would cause the inner side of tires to wear out more rapidly than the mid or outer tire sides. Tesla doesn’t offer a camber arm that is adjustable in order to fix this alignment issue. Due to this the tires blow out prematurely and can potentially cause severe accidents. Tesla doesn’t acknowledge this as a design defect to replace this as safety problem, hence I am reporting it here to investigate the matter.

Potential Consequences:

The MCU screen (large touch screen in the center of dashboard) has been malfunctioning. This started a few months ago when screen seemed to "crash" -- as I'm driving, the screen will turn black and take a long time to come back. On other occasions the map won't load and I receive a notice for "network error" (and have 5g service and not a satellite problem.) This would happen on and off for the past few months. I reported it back in March and was told they would order parts and get back to me and haven't. Then the 12 volt battery died and they replaced it but the MCU problems didn't go away. The last time I drove it, the screen froze up, then turned black and didn't come back on. The collision warning, lane assistance, and adaptive cruise control and steering all stopped working. I COULD engage the auto pilot and cruise control (the notification showed on the small dash board on top of the steering wheel, that they were both engaged) however, they didn't work. I set the car to 65 on the highway and the cruise control wouldn't go over 50. Also, on the back roads, the car drove straight through stop signs without stopping.

The specific issue happen while I was I was pulling out of my driveway. The front passenger side suddenly started squeaking. After getting on the highway and accelerating, the car started shaking, and making louder and louder noises. Inspecting the assembly next to the front passenger side tire, I saw that the control arm assembly was making metal on metal contact and the wheel was not horizontally in place (it wiggles on steering). I took the car to the service center. They confirmed that the control arm assembly is corroded and that the issue could cause an accident if it happens while driving as I could lose steering control.

There was a recall for Tesla Model S and X with MCU1 to have the eMMC card upgrade necessary to keep the unit running or for those that had to upgrade the eMMC or the MCU1 replaced with with an MCU2 before the recall would be reimbursed. I was contacted by Tesla about the reimbursement and I completed the necessary forms. I was later contacted by Tesla and informed I didn’t meet the criteria. I asked for reasons why and they told me to contact my service center. As I have been to the service center 25 times I understand that they will just look at the invoice. So I did this on my own and see that when I had to replace my MCU1 with an MCU2, they listed it as an upgrade. At the time, I didn’t think much of it being listed as an upgrade, but it was the only option communicated to me by Tesla at that time. I had no choice in order to make my Model X safe to drive. Upgrading the eMMC was not an option presented to me. I am requesting that Tesla reconsider my request for reimbursement as I feel I meet the requirements of the recall. Additionally, when I needed to install the MCU2 to make my car safe to drive, the FM/XM radio feature that I paid for when I purchased the car was removed. Six months later the option to add the radio back at my expense was another $500. I am seeking reimbursement from Tesla for the cost of the MCU and radio for a total cost of $3000 plus tax.
