Tesla, Inc. (Tesla) is recalling certain 2024 Cybertruck, 2017-2025 Model 3, and 2020-2025 Model Y vehicles. The tire pressure monitoring system (TPMS) warning light may not remain illuminated between drive cycles, failing to warn the driver of low tire pressure. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 138, "Tire Pressure Monitoring Systems."

Potential Risk:
Driving with improperly inflated tires increases the risk of a crash.

In the event of an unbelted driver, the seat belt warning light and audible chime may not activate as intended. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 208, "Occupant Crash Protection."

Potential Risk:
A seat belt warning system that fails to alert occupants of an unbelted seat belt can increase the risk of injury during a crash.

A factory reset muted the Pedestrian Warning System (PWS) sounds. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 141, "Minimum Sound Requirements for Hybrid and Electric Vehicles."

Potential Risk:
Pedestrians may be unaware of an approaching vehicle if the PWS sounds are muted, increasing the risk of a crash.

In certain circumstances when Autosteer is engaged, the prominence and scope of the feature's controls may not be sufficient to prevent driver misuse of the SAE Level 2 advanced driver-assistance feature.

In certain circumstances when Autosteer is engaged, and the driver does not maintain responsibility for vehicle operation and is unprepared to intervene as necessary or fails to recognize when Autosteer is canceled or not engaged, there may be an increased risk of a crash.

Safety Issue:

Tesla, Inc. (Tesla) is recalling certain 2016-2023 Model S, Model X, 2017-2023 Model 3, and 2020-2023 Model Y vehicles equipped with Full Self-Driving Beta (FSD Beta) software or pending installation. The FSD Beta system may allow the vehicle to act unsafe around intersections, such as traveling straight through an intersection while in a turn-only lane, entering a stop sign-controlled intersection without coming to a complete stop, or proceeding into an intersection during a steady yellow traffic signal without due caution. In addition, the system may respond insufficiently to changes in posted speed limits or not adequately account for the driver's adjustment of the vehicle's speed to exceed posted speed limits.

Potential Risk:
FSD Beta software that allows a vehicle to exceed speed limits or travel through intersections in an unlawful or unpredictable manner increases the risk of a crash.

Safety Issue:

Tesla, Inc. (Tesla) is recalling certain 2019-2021 Model 3 and 2020-2021 Model Y vehicles. The brake caliper bolts may be loose, allowing the brake caliper to separate and contact the wheel rim.

Potential Risk:
Contact with the rim may cause a loss of tire pressure, increasing the risk of a crash.
FREE Recall Solution:
Tesla Service will inspect and tighten, or replace the caliper bolts as necessary, free of charge. Owner notification letters were mailed July 23, 2021. Owners may contact Tesla customer service online by visiting www.tesla.com/support/contact or by calling 1-877-79-TESLA (or 1-877-798-3752). Tesla's number for this recall is SB-21-33-002.

Safety Issue:

Tesla, Inc. (Tesla) is recalling certain 2018-2020 Model 3 and 2019-2021 Model Y vehicles. One or both fasteners that secure the front seat shoulder belt to the b-pillar may not be properly attached.

Potential Risk:
An improperly attached fastener may prevent the seat belt system from performing as designed, increasing the risk of injury.
FREE Recall Solution:
Tesla Service will inspect and repair both fasteners if necessary, free of charge. Owner notification letters were mailed July 23, 2021. I AM STILL WAITING FOR TESLA TO MAKE THIS REPAIR. I HAVE CONTACTED A SERVICE CENTER SEVERAL TIMES AND TRIED TO MAKE AN APPOINTMENT ONCE. TESLA CANCELLED THE APPOINTMENT AND KEEPS TELLING ME THEY HAVE NOT RECEIVED THE PARTS TO MAKE THE REPAIR. IT HAS BEEN OVER A YEAR AND A HALF SINCE TESLA ANNOUNCED THE RECALL AND IT HAS FAILED TO REPAIR MY CAR. MY UNDERSTANDING IS THAT THE HARNESS CAN SHORT OUT AND CAUSE A FIRE.

After receiving fault RCM_a094 I discovered that the seatbelt wiring harness had detached. When I took the vehicle in for repair, I was told it was caused by the wire being "tugged" on. Since I have never used the seatbelt in any capacity other than how it was intended, I believe this is a design flaw with the restraint system.

SERVICE BRAKES

Potential Consequences:

Braking and Stopping - Braking system - Improperly functioning braking systems. I experienced a problem with the brake pedal, brake calipers, and/or any component of a Model 3 braking system. “Model 3 has an anti-lock braking system (ABS) that prevents the wheels from locking when you apply maximum brake pressure. This improves steering control during heavy braking in most road conditions…Forward Collision Warning and Automatic Emergency Braking do not operate when driving at very low speeds (see Collision Avoidance Assist on page 128). Do not rely on these features to warn you, or to prevent or reduce-the impact of a collision. (Tesla owners Manuel71-73) I stop on a downward slope with my foot on the brake to wait for a gate to open in my apartments. I let go of the brake and the vehicle remained steady until it started rolling. When it started rolling, I jam my foot on the brake but the brake is engaged (locked). The vehicle accelerates, the brakes and steering wheel is locked giving no control over the vehicle. The vehicle drives through the gate and into a wall. The airbags deploy and I get out immediately for caution of explosion and shock. I call a tow truck and made a claim with USAA. I got a concussion and multiple injuries on my body. I’ve been getting medical treatment for 6-8 months. USAA didn’t help as they deemed the accident was my fault and totaled the vehicle. I bought the vehicle for more than what’s it’s worth. I have loan payment that I can NOT pay. This was my only asset and this accident changed my work life. Tesla gave me the run around on how to make a claim and no compensation. Directed me to insurance company. This demonstrates failure of an Braking System, operating electric vehicles, company responsibility, safety and transportation regulations and policies. These vehicles are highly dangerous for my safety and others as there’ve been multiple accidents and deaths.

The on screen messages stated “Gear Box Fluid Recommended” and “Adaptive Air Suspension Needs Service”. When I reached out to Tesla they told me to not worry about those alerts because a bug in their software update caused the issue and I did not actually require those services. They admitted that they were aware of this bug and that I was not the only customer experiencing this issue. They gave me two options. Either schedule a service appointment or wait until the next software update. I scheduled an appointment because I could not drive my vehicle safely with alerts beeping every second that I was driving. A software update appeared on my screen before my appointment and it seemed to fix the problem for about 24 hours. The next day I woke up to my screen being black and the car not being responsive or displaying any information. This caused a major concern because I could not even tell how fast I was driving. The car is still driveable. I just have no information or access to any functions of the vehicle. So I scheduled another appointment with Tesla. Who determined the entire computer system needed to be replaced. Since my car is out of warranty they want me to pay out of pocket. I do not believe this is a responsibility of mine since the software update is what destroyed my computer. Tesla is responsible for making sure their vehicles are safe to drive and providing us with software updates that make the cars extremely unsafe to drive and then not wanting to be responsible for the repairs is insane.
