Tesla, Inc. (Tesla) is recalling certain 2023 Model 3 and Model Y vehicles operating software prior to 2023.38.4. The printed circuit board for the electronic power steering assist may experience an overstress condition, causing a loss of power steering assist when the vehicle reaches a stop and then accelerates again.

Potential Risk:
A loss of power steering assist can require greater steering effort, especially at low speeds, increasing the risk of a crash.

Safety Issue:

Tesla, Inc. (Tesla) is recalling certain 2024-2025 Model 3, Model S, 2023-2025 Model X, and Model Y vehicles. The computer circuit board may short, resulting in the loss of the rearview camera image. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."

Potential Risk:
A rearview camera that does not display an image reduces the driver's rear view, increasing the risk of a crash.
FREE Recall Solution:
Tesla released an over-the-air (OTA) software update, free of charge. Tesla will also identify any vehicles that experienced a circuit board failure, or stress that may lead to a circuit board failure, and replace the affected computers, free of charge. The tire pressure monitoring system (TPMS) warning light may not remain illuminated between drive cycles, failing to warn the driver of low tire pressure. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 138, "Tire Pressure Monitoring Systems."

Driving with improperly inflated tires increases the risk of a crash.

Tesla, Inc. (Tesla) is recalling certain 2016-2023 Model S, Model X, 2017-2023 Model 3, and 2020-2023 Model Y vehicles equipped with Full Self-Driving Beta (FSD Beta) software or pending installation. The FSD Beta system may allow the vehicle to act unsafe around intersections, such as traveling straight through an intersection while in a turn-only lane, entering a stop sign-controlled intersection without coming to a complete stop, or proceeding into an intersection during a steady yellow traffic signal without due caution. In addition, the system may respond insufficiently to changes in posted speed limits or not adequately account for the driver's adjustment of the vehicle's speed to exceed posted speed limits.

FSD Beta software that allows a vehicle to exceed speed limits or travel through intersections in an unlawful or unpredictable manner increases the risk of a crash.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL

Potential Consequences:

I was fully stopped at red light of intersection of [XXX] . Suddenly my vehicle accelerate it self and start auto drive at high speed and malfunction, hitting multiple parked vehicles. My vehicle was on auto holding brake feature. I was not able to control my vehicle, even when I tried pressing the brake. No warnings or alarms from my vehicle when crashes into multiple parked cars. Corrective Action:

Defect Description:

SUSPENSION

Potential Consequences:

The contact owns a 2023 Tesla Model Y. The contact stated while driving at various speeds, there was an abnormal scraping sound from the vehicle. There was no warning light illuminated. Upon inspection, the contact discovered that a bolt was hanging off the front passenger’s side of the suspension. Upon further inspection, the contact discovered that two additional bolts on the bracket were also loosened. The contact was able to tighten the bolt to secure the suspension. The contact called a Tesla Service Center but received no assistance. The manufacturer was not notified of the failure. The vehicle was not diagnosed or repaired. The failure mileage was approximately 49,000.

Defect Description:

Potential Consequences:

Emergency braking and alerting engaged. There were no vehicles or objects in front of my vehicle at the time. The braking was hard enough to engage the ABS and bring the car from 40mph to 5-10 mph, risking a rear end collision if someone was behind me. I have owned the car just over 1 year, this is the first time this has happened. No warnings or other errors appeared before the incident and no errors are present after the incident. Neither the dealer or anyone else has inspected the vehicle. Conditions: Dry, Night (9:50pm) no cruise control or lane keeping functions enabled, no cars or objects were seen in front of the vehicle.

I noticed that the car window glass broke without any external impact, and there are no signs of external damage. I suspect this may have been caused by stress fractures due to improper assembly. From what I understand, stress fractures often occur at the edges without any signs of external impact, which matches this situation. But Tesla service refused to admit this is a flaw of installation and refuse to replace it under the warranty coverage. After exploration, I found there are plenty of similar cases happened on the internet. And some of them were treated as flaw, some not. How Tesla identify this kind of case as flaw or not? They didn't even check my out in person. I cannot accept that!

Front left suspension came apart without any collision. Over the course of a couple days, noticed a sound in the suspension when turning the steering wheel to make a 90 degree turn or more. Pulling out of a driveway and hear loud sound and then steering wheel would not move the front left tire. After getting out to inspect what happened, noticed there were two large bolts on the ground and a large piece of the suspension was hanging down under the car and sitting on the ground. There were no warnings prior to this occurring to signal that something was loose or malfunctioning. Luckily I was on a quiet residential road when it happened and was able to get out safely but it would have been extremely dangerous if this happened in a different location.

At approximately, 14,300 miles and while still under full manufacturer's warranty coverage, the front end of my vehicle started vibrating excessively at speeds of over 65 MPH while driving on smooth road surface (highway driving). I took the car to the local TESLA dealership to have the problem diagnosed. They determined that the "foam lining" on the inside of the tire separated and caused the wheel to become unbalanced. Their fix was to remove the foam lining and reinstall the tire on the wheel without the foam lining. I now have 1 tire out of 4 that does not meet the original OEM spec and condition. Below are the notes from the invoice for $256.10 that I had to pay to correct the problem. Repair Notes: Verified customer concern. Found RH front tire to have foam separation. Removed foam rebelanced tires. Test drove vehicle to verify vibration no longer present. Verified repairs.Correction: Tire - Front - RH (Remove & Install) - Remove and Install This seems to be a common problem with these foam lined tires manufactured by GOODYEAR according to TESLA owner blogs. It seems that either TESLA and or GOODYEAR are not claiming responsibility for a potential design and or manufacturing problem which is why I am filing a formal complaint. I will also be filing a complaint specific to the GOODYEAR tire.

Potential Consequences:

At approximately, 14,300 miles and while still under full manufacturer's warranty coverage, the front end of my vehicle started vibrating excessively at speeds of over 65 MPH while driving on smooth road surface (highway driving). I took the car to the local TESLA dealership to have the problem diagnosed. They determined that the "foam lining" on the inside of the tire separated and caused the wheel to become unbalanced. Their fix was to remove the foam lining and reinstall the tire on the wheel without the foam lining. I now have 1 tire out of 4 that does not meet the original OEM spec and condition. Below are the notes from the invoice for $256.10 that I had to pay to correct the problem. Repair Notes: Verified customer concern. Found RH front tire to have foam separation. Removed foam rebelanced tires. Test drove vehicle to verify vibration no longer present. SUSPENSION

Potential Consequences:

I found that two bolts that connect the front right suspension assembly to the body of the car were loose (actually one had completely come off). On 9/12 I heard a rather loud clanging sound when I accelerated. When I arrived home I looked under the car, saw those loose bolts, and took the two pictures that I attached, which were taken from in front of the behind the front right wheel. We bought this car new from Tesla, and received it in late August 2023. We would expect that those bolts were not properly tightened when the car was assembled at the factory, since servicing done afterwards should not have affected them. This is the list of servicing that was done after we received the car, and before 9/12: - Tesla service fixed some rattling sounds that we heard inside the car by adjusting the interior trim, and by fixing something in the brake pedal that was making some rattling sound sometime (replaced the brake pin, insulate the braked pedal and lubricated the brake pin) - A body shop replaced the side mirror on the left side (i.e. opposite side of the bolts that had come off), which had been hit by another car - A tire shop fixed a hole in a tire and rotated the tires
