Tesla, Inc. (Tesla) is recalling certain 2024 Cybertruck, 2017-2025 Model 3, and 2020-2025 Model Y vehicles. The tire pressure monitoring system (TPMS) warning light may not remain illuminated between drive cycles, failing to warn the driver of low tire pressure. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 138, "Tire Pressure Monitoring Systems."

Potential Risk:
Driving with improperly inflated tires increases the risk of a crash.

A factory reset muted the Pedestrian Warning System (PWS) sounds. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 141, "Minimum Sound Requirements for Hybrid and Electric Vehicles."

Pedestrians may be unaware of an approaching vehicle if the PWS sounds are muted, increasing the risk of a crash.

In certain circumstances when Autosteer is engaged, the prominence and scope of the feature's controls may not be sufficient to prevent driver misuse of the SAE Level 2 advanced driver-assistance feature.

In certain circumstances when Autosteer is engaged, and the driver does not maintain responsibility for vehicle operation and is unprepared to intervene as necessary or fails to recognize when Autosteer is canceled or not engaged, there may be an increased risk of a crash.

Tesla, Inc. (Tesla) is recalling certain 2016-2023 Model S, Model X, 2017-2023 Model 3, and 2020-2023 Model Y vehicles equipped with Full Self-Driving Beta (FSD Beta) software or pending installation. The FSD Beta system may allow the vehicle to act unsafe around intersections, such as traveling straight through an intersection while in a turn-only lane, entering a stop sign-controlled intersection without coming to a complete stop, or proceeding into an intersection during a steady yellow traffic signal without due caution. In addition, the system may respond insufficiently to changes in posted speed limits or not adequately account for the driver's adjustment of the vehicle's speed to exceed posted speed limits.

Potential Risk:
FSD Beta software that allows a vehicle to exceed speed limits or travel through intersections in an unlawful or unpredictable manner increases the risk of a crash.

Swapped camera views can prevent the vehicle's cameras from detecting crossing objects and high-curvature lane lines, impairing the Autosteer and Automatic Emergency Braking (AEB) performance, and increasing the risk of a crash.

Potential Risk:
Contact with the rim may cause a loss of tire pressure, increasing the risk of a crash.
FREE Recall Solution:
Tesla Service will inspect and tighten, or replace the caliper bolts as necessary, free of charge. Owner notification letters were mailed July 23, 2021. The contact owns a 2021 Tesla Model Y. The contact stated that while her husband was driving approximately 75 MPH with the cruise control activated, the vehicle experienced phantom braking. Defect Description:

FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING

Potential Consequences:

Frequent, random, phantom braking using cruise control. Informed Tesla with times +/- one minute. Sometimes the braking was very hard and could cause a rear-end collision (at highway speeds). Always occurred with no warning and highly variable conditions (light, or hills, or overpasses, and then apparently nothing). I called Tesla and technician said overpasses were a problem. Vehicle and components have not been checked.

Corrective Action:

Potential Consequences:

I have an ongoing problem with the dashcam recording, which for me is a safety issue. The dashcam constantly records from several cameras what is happening around the vehicle. The dashcam icon will show a red “x,” indicating that the dashcam is not working. Probably 80% of the time I get in the car, the red x is there. The solution involves opening the glove box and pulling the drive, waiting for up to 20 seconds, then replacing it. If that doesn’t work, the usb drive needs to be formatted and all previous clips are lost, but probably 95% of the time the reformat fails. It was recommended by Tesla service that I purchase another usb drive ($45?), but that did not solve the problem. I’ve had my car into Tesla service for this issue or reported it through the app multiple times. For months, I’ve been told it will be addressed with a firmware update, but it has not happened. Other Tesla owners are also having this issue, but it was addressed with a firmware update and the problem was fixed. The owners who have the Full Self Driving Beta have NOT had a firmware update to address the issue. We are the owners who paid $10-15,000 extra for the full self driving feature. I have not been able to get any information from Tesla about an expected timeframe for this update. I ask, but I’m always told it will be a “future update.” I have asked if anyone is working on this issue, and they won’t answer that question either. I would like to know the status. I’ve been very patient, but since there is aggression in the areas where I drive against EV drivers, and Teslas in particular, I need this feature to be working. I have been harassed, had large pickup drivers tailgate or try to run me off the road, and I’ve been coal rolled. I’ve already used data from the dashcam to report aggressive drivers to the police.

On January 26, 2022, I had 47 episodes, in just a couple of hours. This is typical. Several of the phantom braking episodes occurred with vehicles tailgating me. This is a grave safety issue and requires immediate attention. My Tesla Model Y has been to the Tesla service center over 4 times regarding this problem without resolution. They say phantom braking is a software issue and only the engineers can fix it. I was assured soon after delivery that the software would be updated quickly, and it would fix the problem. The issue still exists. I am very disappointed with the vehicle and fear for my safety. Please require Tesla to remedy this very serious safety issue immediately.
