Potential Risk:
An inoperative horn may prevent the driver from alerting other drivers, increasing the risk of a crash.

As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) numbers 208, "Occupant Crash Protection" and 212, "Windshield Mounting."

Potential Risk:
A torn air bag may not adequately protect an occupant in a crash, increasing the risk of injury.

An incorrect font size is displayed on the instrument panel for the Brake, Park, and Antilock Brake System (ABS) warning lights. In certain circumstances when Autosteer is engaged, the prominence and scope of the feature's controls may not be sufficient to prevent driver misuse of the SAE Level 2 advanced driver-assistance feature.

In certain circumstances when Autosteer is engaged, and the driver does not maintain responsibility for vehicle operation and is unprepared to intervene as necessary or fails to recognize when Autosteer is canceled or not engaged, there may be an increased risk of a crash.

(Tesla) is recalling certain 2016-2023 Model S, Model X, 2017-2023 Model 3, and 2020-2023 Model Y vehicles equipped with Full Self-Driving Beta (FSD Beta) software or pending installation. The FSD Beta system may allow the vehicle to act unsafe around intersections, such as traveling straight through an intersection while in a turn-only lane, entering a stop sign-controlled intersection without coming to a complete stop, or proceeding into an intersection during a steady yellow traffic signal without due caution. In addition, the system may respond insufficiently to changes in posted speed limits or not adequately account for the driver's adjustment of the vehicle's speed to exceed posted speed limits.

Potential Risk:
FSD Beta software that allows a vehicle to exceed speed limits or travel through intersections in an unlawful or unpredictable manner increases the risk of a crash.

The Boombox function allows sounds to be played through an external speaker while the vehicle is in motion, which may obscure the Pedestrian Warning System (PWS) sounds. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 141, "Minimum Sound Requirements for Hybrid and Electric Vehicles."

Potential Risk:
Pedestrians may be unaware of an approaching vehicle if the PWS sounds are obscured, increasing the risk of a crash.

Tesla will perform an over-the-air (OTA) software update that disables the "rolling stop" functionality, free of charge. Owner notification letters were mailed March 28, 2022. Owners may contact Tesla customer service at 1-877-798-3752. Tesla's number for this recall is SB-22-00-001.

Dangerous Phantom Braking: this has occurred many times including very dangerous situations in high speed interstate hwy settings. 1) Lower drivers knee airbag panel improperly installed. Photos available. 2) This could cause malfunction of the airbag in an accident 3) No response from Tesla service when reported in the Tesla App, and in person. 4) No, but I have photos and the EPC identifies the parts in question. 5) Loose trim panel made the problem apparent, noted at delivery. Noted loose and broken lower dash trim panel under the steering Yoke. Upon investigation, noted the panel was designed to be held in place by 4 circular magnets, two on the panel and two on the lower dash support. However, in this case, a 10mm high strength steel bolt with a torque stripe was inserted into the mating magnet on the lower dash structure. The strength of the magnet was such that the bolt was apparently fully seated. The problem was, where did this bolt belong? It was found the adjacent lower panel was the knee airbag panel was designed to be secured by two 10mm bolts, and one was missing! The errant bolt was placed into the proper hole and torqued per the SM. The passenger side panels and bolts were found to be properly installed, and torque striped. I believe the placement of the Yoke makes it difficult for assembly personnel to ensure the airbag bolt is placed into the proper hole, and the magnet keeps the bolt in position when it is incorrectly placed. The problem is this leaves the lower knee airbag secured by a single bolt, and that is a potential safety concern. Please contact me for photos and supporting documents.

Brought into Tesla dealer and they claim it was from an external windshield chip that then spread although no external chip was present. They seemed very knowledgeable about this issue and trained to try to not cover this under warranty.

Potential Consequences:

The contact owns a 2022 Tesla Model S. The contact stated while her husband was driving approximately 70 MPH, the vehicle experienced "phantom braking" approximately 8-times in total. The contact stated no warning light was illuminated. The contact called the local dealer and made them aware of the failure. The vehicle was not diagnosed or repaired. The manufacturer had not been informed of the failure. The failure mileage was approximately 2,000.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL

Potential Consequences:

Malfunctioning system: AUTOPILOT, Self Driving. The vehicle is available for inspection. We were put at RISK following having several 'NEAR MISS' incidents. None of which resulted in an accident. The 2022 Model S PLAID with less than 2,929 current miles as of this entry, while being driven in daylight & evening hours on major freeways and interstate roads in Central & Southern California while using the "AUTOPILOT" FULL SELF DRIVING (FSD) has multiple times for NO APPARENT REASON & WITHOUT WARNING suddenly 'PANIC' BRAKED the vehicle from freeway speeds to less than 30 MPH. In one instance this caused the late model vehicle following us to take immediate evasive action and swerve into the adjacent lane without warning and then the vehicle HONKED continuously at us while passing us to our left. On each of the occasions I have had to OVERRIDE the Autopilot by pressing on the accelerator pedal as soon as I realized what was happening! On other occasions when in 'AUTOPILOT' self-driving mode, the vehicle has attempted to change lanes to exit the highway without awareness of how close the vehicle was to the end of the exit lane. Again I had to OVERRIDE the Autopilot by turning the yoke back into the lane to avoid running into barriers. I had believed that because this is a 'BETA' version that these conditions were being worked on and would be corrected with a software update. I have had numerous software updates installed. The current Version is 2022.44.30.10. For this reason, I have not reported these incidents to the manufacturer or made an appointment with the Local Tesla Service Center. The problems continue even with the current software update. Until recently, I was unaware of others having these same issues or that significant injury, death, damage, have occurred. However, since being made aware of this problem, I chose to report my experiences. I will follow-up and make an appointment with the manufacturer to have the vehicle inspected.
