The FSD Beta system may allow the vehicle to act unsafe around intersections, such as traveling straight through an intersection while in a turn-only lane, entering a stop sign-controlled intersection without coming to a complete stop, or proceeding into an intersection during a steady yellow traffic signal without due caution. In addition, the system may respond insufficiently to changes in posted speed limits or not adequately account for the driver's adjustment of the vehicle's speed to exceed posted speed limits.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
Potential Consequences:
The contact owns a 2021 Tesla Model 3. The contact stated while using a self-driving mode at 50 mph, it would not stay in the lane and drive slowly into the next lane. The contact stated on April 9, 2023, while using a self-driving mode at 15 mph, a pedestrian walked into the lane and the vehicle did not detect the person and hit the pedestrian. No air bags deployed. The contact did not sustain any injuries. The pedestrian sustained an unknown fractured leg and a fractured arm and sought medical attention for his injuries. A police report was filed. The vehicle was not drivable. The vehicle was towed to a Tesla collision shop and had the front windshield replaced. The contact took the vehicle to the local dealer but was informed no failure was detected. The vehicle was not repaired and continued to experience the failure. The manufacturer had been informed of the failure. The failure mileage was 48,570.

There is a design defect in the Tesla Model 3 which causes moisture to accumulate in the air filters. The filters then sit wet and develop mold and mildew. The first replacement was needed within the first 2 months of vehicle ownership. When the heat / air is turned into the on position, this fowl air is distributed to passengers in an enclosed environment and it becomes a health hazard, especially to those with preexisting respiratory issues. This is an ongoing problem with the soiled air filters requiring constant replacement. A Tesla mobile service agent confirmed that Tesla engineering team is aware of the issue, but hasn't come up with a permanent solution. The service agent stated they are spending tens of thousands of dollars in replacements filters and that a majority of his service calls are to replace the mildewed filters. The problem is Tesla continues to charge people to replace the filter with no solution for fixing the problem.

FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
Potential Consequences:
Multiple phantom breaking events while driving at highway speed. Luckily there were no cars behind us, otherwise there could have been a collision. Caused driver and passenger whiplash and items in car to be forcibly displaced. There were no advance warning. Tried to report to manufacturer but they seem to be closed for the holiday, will try again to report after holiday.
Corrective Action:
