As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard numbers 214, "Side Impact Protection" and 110, "Tire Selection and Rims."

Potential Consequences:
The contact owned a 2021 Ram 3500 Promaster. The contact stated one of his employees was driving at 40 MPH, when a burning odor was emitted from the vehicle. Smoke was emitting from under the hood and entered the vehicle through the A/C vents. The driver exited the vehicle. The driver did not sustain any injuries. None of the air bags deployed. It was unknown if any warning lights were illuminated. A police report was filed. The vehicle was consumed by the fire and destroyed. The vehicle was towed to a towing yard. The origin of the fire was the engine. The manufacturer was not notified of the failure. The contact notified the insurance company. The failure mileage was approximately 80,000.

SERVICE BRAKES
Potential Consequences:
My van began making a loud squeaky sound, louder at low speeds and less at higher speeds. It sounded like it was coming from the wheels or undercarriage. There were no dash warning lights, alarms, or any other indicator of a serious issue. I drove the van around town for a few days approximately 30 miles. The van is heavy and I live in a hilly area, so I use the parking brake fairly often. I applied the parking brake within that week on an incline, and when I released the parking brake and drove forward, the squeak was louder then ever, so much so, people stared. There were no error or warning message on the dash or elsewhere. The parking brake indicator had come on normally while engaged and went off when it was released. I drove it directly to the local Ram dealer, as it seemed worse, hoping it was just in need of minor lubrication. It had just been serviced at the same dealer a few months earlier and I was told "it was like new with its low 12,500 miles". I explained the new noise and that I didn't want to cause damage by driving it, although there were no warning lights on. My van is still under the original warranty for a few more months. Even though they were booked, an employee went out and felt by the rear driver's side wheel and said it was really hot and likely a frozen brake caliper. They were able to fit me in and discovered the parking brake was frozen, which was causing the noise. They ordered the parts to repair, disengaged the parking brake, and said it was safe to drive as the main brakes were a separate system. They said not to apply the parking brake again as it would hang up as before. -The dealer ordered the parts over a month ago, but said the brake parts are not only on back order, but with no estimated shipping date, meaning they aren't even manufacturing them. -I am driving without a parking brake in a heavy vehicle in a hilly area with no pending solution and heading into winter weather. This is a safety issue! Please help ASAP.

While preparing for a trip, I noticed the upper kitchen cabinetry is detaching from the ceiling and in danger of falling. This section is part of the cabinetry housing the microwave. The detaching cabinet has a glass plate attached to the bottom as a heat guard for the stove directly below it. The stove also has a glass top. It has the potential to injury an occupant both by its weight and glass breakage during travel or when stationary. Occupants can be sitting directly across from the cabinet on the couch which has seat belts. There is a 16 inch aisle between the stove top and the edge of the couch. I'm not sure if this is the appropriate type of safety issue for your agency, but I felt it could surely injure occupants in the kitchen area.

The component that failed/malfunctioned is the CV axle, the part is no longer available for inspection. The safety risk posed is loss of control of the vehicle, subsequent damage to other vital steering and stability parts such as the wheel bearings, axles, alignment, and lower ball joint. The problem has been confirmed by a RAM dealership, and corrected under an extended warranty. The component has not been inspected outside of the RAM dealership technicians and potentially the warranty provider. There were no warning lights or indicators from the computer/OBD. Physical symptoms and signs included uncontrollable vehicle shaking/vibration, and difficulty maintaining control at high speeds. Other symptoms/signs may include the frequent need for realignment, and unique tire tread wear patterns. Symptoms appeared around 58,000 original miles. With a fleet of these vans, this problem has been noted more than once.

Corrective Action:
I had to replace my backup camera as it was faulty and the newer models had a recall. My blind spot monitoring system cuts off randomly going down the road. We have paid out of pocket for a new camera and the dealership can't figure out why the blind spot indicator cuts off.

Potential Consequences:
The driver was driving uphill when his RAM ProMaster suddenly lost power and rolled back down the hill, hitting a tree and stopping. During this time, the driver had absolutely no control over the vehicle, and the brakes didn’t work at all. The driver attempted to steer the vehicle, pumped the brakes, turned the key, and pulled the emergency brake, but nothing worked. Internal and external cameras captured the entire event. When we started the engine, the awning deployed and almost injured one of us. We have already supposedly had this problem repaired due to a recall. Obviously it is still a danger to ourselves and others.
