POWER TRAIN
Potential Consequences:
The transmission slips or doesn't engage appropriately. The van lurches forward at intersections and slips, pauses with a loud clunking noise at lower gears when driving in town. This morning, I attempted to cross a double highway...pressed the accelerator, nothing happened, then the vehicle lurched forward, stopped again, then lurched forward again all with the accelerator pressed down. I was almost hit broadside while crossing the highway. This is not the first time this has occurred. Multiple people in our household drive this vehicle and all have reported the same concern. I've asked about this every time I get routine maintenance but Honda dealership says it's working as designed and they've witnessed this issue in other customer reports. This is a significant safety risk!

Defect Description:
FORWARD COLLISION AVOIDANCE: WARNINGS
Potential Consequences:
On January 18, 2024 I had the following recalls performed on 2019 Honda Odyssey Elite: Auto Idle Stop and the Media Oriented Systems Transport (MOST) communication coaxial cable. Immediately after the recall work was completed, I began receiving an error code "Collision Mitigation System Problem". The error code would flash after this sequence of events: After my car's engine has come to temperature and is warm. I come to a complete stop, my car goes into Idle Stop and when I release the brake to accelerate and the engine re-engages, this error flashes. And, I can continue to drive. Prior to this recall work, I had never received this error. In addition to this error code, on January 24, 2024, I was at a complete stop on a freeway offramp. When I released the brake and tried to accelerate, my car would not engage and very loud clunk, followed by the orange "Brake System" light flashed on. Eventually my car engaged and we were able to drive. The clunk was so loud my teenage children asked me if we were hit by another car. On, Friday January 25, 2024 after dropping the car off at the dealership we were told that "The diagnostics have revealed a 'defective Multipurpose Camera and AIM'. And, the cost to replace is $1793.77 plus tax.

Defect Description:
ELECTRICAL SYSTEM
Potential Consequences:
In early December 2023, while driving, I experienced a sudden malfunction in my LKAS system, receiving a notification that it had stopped working without any prior indications. In response, I promptly pulled over, turned off the vehicle, and restarted it, hoping to clear any error messages. Unfortunately, the issue persisted. Subsequently, I took my vehicle to the Honda service center, where I was informed that the front camera was malfunctioning, triggering multiple error messages such as collision system problems, auto high beam issues, and adaptive cruise control malfunctions—all of which rendered these safety features inoperable. The service center clarified that, while it was deemed safe to drive the vehicle, the automatic braking system and front camera features were unavailable, and any necessary repairs would be at my expense. The paperwork provided indicated the discovery of DTC U3000-49, citing a multipurpose camera unit/millimeter wave radar internal electronic failure. Additionally, beyond the LKAS system issue, I have repeatedly encountered difficulties with my vehicle's acceleration. Notably, there is a struggle to accelerate from a complete stop, and a significant delay in acceleration occurs when removing my foot from the gas pedal, especially in the middle of intersections. This delay has resulted in near collisions, with other vehicles honking and, at times, coming dangerously close to colliding with my vehicle. Despite reporting this ongoing acceleration problem to the dealer multiple times, the issue persists and has worsened over time. This comprehensive incident report aims to document and address the technical malfunctions in my vehicle, emphasizing the urgent need for a thorough examination and resolution of these safety concerns.

Additional Notes:
In early December 2023, while driving, I experienced a sudden malfunction in my LKAS system, receiving a notification that it had stopped working without any prior indications. In response, I promptly pulled over, turned off the vehicle, and restarted it, hoping to clear any error messages. Unfortunately, the issue persisted. Defect Description:
UNKNOWN OR OTHER
Potential Consequences:
On June 2023, the driver side rear sliding door of my Honda Odyssey abruptly ceased functioning. Despite the infrequent use of this door—since I typically rely on the passenger side for dropping off my three children at school and activities—it is perplexing that the underused door has encountered such a critical malfunction. Given that our family of five heavily depends on this vehicle for transportation, the inability to utilize both doors has not only caused significant inconvenience but also raised substantial safety concerns. The driver side rear sliding door is currently sealed shut and attempts to open it manually have proven futile. It appears that the latching mechanism has completely failed, as the motor makes an audible attempt to open the door, yet no actual movement occurs. This issue has persisted for over six months, prompting ongoing communications with Norm Reeves Honda in West Covina and Honda's customer service team. According to the Honda report, it is suggested that the door may have been forced open. The estimated cost for potential repairs is $1,679.19, pending a proper assessment of the specific problem once access to the affected area is granted. The technician mentioned the necessity of breaking the paneling to reach the latches for replacement. Notably, this is my second Honda Odyssey, and two other family members also own Odysseys. Honda's van doors have a documented history of malfunctions, and it is disheartening to observe the lack of attention to this recurring issue. Moreover, the burden of the associated costs is unfairly placed on the customer. The prolonged delay in resolving this matter and the financial implications underscore a pressing need for Honda to address and rectify these door malfunctions promptly and responsibly.

ELECTRICAL SYSTEM
Potential Consequences:
Over the years of owning this vehicle, we've encountered sporadic electrical problems that have led to the car either stalling or needing a jump start. Despite these issues, inspections of the battery have revealed no obvious problems. Additionally, we've had intermittent system warning messages for features such as Adaptive Cruise Control, Forward Collision Warning, and Lane Departure. These warnings have surfaced and vanished on multiple occasions. The vehicle has never been in an accident or experienced anything beyond normal operating conditions.

LANE DEPARTURE: WARNING
Potential Consequences:
There has been no incident. And I hope there is not. But the forward camera that assists in the road departure mitigation, forward collision mitigation and lane keeping assist has failed. Also inoperative is the cruise control, auto dimming headlamps and the brake system light is on. I find it hard to believe that after some of the less critical recalls that have been sent out for this vehicle that they don’t address this very important safety system. The camera alone costs $800 and that doesn’t include the cost of installation and calibration. I have a family of six and we use this van extensively. But we cannot afford this repair and are forced to drive with all of these lights on in the instruments. We take extremely good care of our vehicles. It is serviced well and has been in no major accidents. Please help us get action for ourselves and others who are in danger
