The printed circuit board for the electronic power steering assist may experience an overstress condition, causing a loss of power steering assist when the vehicle reaches a stop and then accelerates again.

Potential Risk:
A loss of power steering assist can require greater steering effort, especially at low speeds, increasing the risk of a crash.

Safety Issue:

Tesla, Inc. (Tesla) is recalling certain 2024-2025 Model 3, Model S, 2023-2025 Model X, and Model Y vehicles. The computer circuit board may short, resulting in the loss of the rearview camera image. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."

Potential Risk:
A rearview camera that does not display an image reduces the driver's rear view, increasing the risk of a crash.

Tesla released an over-the-air (OTA) software update, free of charge. Tesla will also identify any vehicles that experienced a circuit board failure, or stress that may lead to a circuit board failure, and replace the affected computers, free of charge. The tire pressure monitoring system (TPMS) warning light may not remain illuminated between drive cycles, failing to warn the driver of low tire pressure. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 138, "Tire Pressure Monitoring Systems."

Driving with improperly inflated tires increases the risk of a crash.

(Tesla) is recalling certain 2016-2023 Model S, Model X, 2017-2023 Model 3, and 2020-2023 Model Y vehicles equipped with Full Self-Driving Beta (FSD Beta) software or pending installation. The FSD Beta system may allow the vehicle to act unsafe around intersections, such as traveling straight through an intersection while in a turn-only lane, entering a stop sign-controlled intersection without coming to a complete stop, or proceeding into an intersection during a steady yellow traffic signal without due caution. In addition, the system may respond insufficiently to changes in posted speed limits or not adequately account for the driver's adjustment of the vehicle's speed to exceed posted speed limits.

ELECTRICAL SYSTEM:ADAS:AUTONOMOUS/SELF DRIVING

Potential Consequences:

The contact owns a 2023 Tesla Model Y. The contact stated while driving 75 MPH with Self Driving feature activated, the vehicle experienced phantom braking and abruptly decelerated to 35 MPH. The contact stated that there was no vehicle or object in front of the vehicle. The contact stated that the failure reoccurred several times. The contact stated that the initial failure occurred after a software update was remotely completed the day before. The manufacturer was not notified of the failure. The vehicle was not diagnosed nor repaired. The failure mileage was approximately 4,500. The consumer stated the manufacture was notified on 04/17/23.

In all three cases, the car was on adaptive cruise control at about 70 miles per hour. There was no car within 200 yards. The car suddenly braked and the speed went down to 30 miles per hour in a couple of seconds. I had to intervene and cancel the adaptive cruise control.

I was driving on a 4-lane road with no traffic, then the car suddenly reduced its speed from 65 mph to almost 30 mph, causing a sudden jolt to everyone in the car. There was nothing in front or reason for the car to break. No incoming traffic. It does not seem to have detected a "ghost" obstacle as an emergency. As there was no beeping or emergency message on display. No warning of any type. Luckily, no car was following me; otherwise, it could have caused a rear-end collision.

The incident happened on 4/9/2023 at 2:30 pm. I drove in I-10 from Phoenix to California with Cruise Control and Autosteer ON. The speed was set to the highway speed limit. There were three other passengers with me. This seems to be related to the known issue of "Phantom Breaking" in Teslas. I had the car for exactly one week. I was driving the vehicle in the street, and my vehicle jerked and stopped in the middle of the intersection and a warning sign came on “rear motor disabled”. Then stated it two or three more times, stating the vehicle is safe to drive under 50 miles, and no steel grades. I do not feel safe driving this vehicle at all at this time, and feel that Tesla needs to replace my vehicle as the vehicle is a week old, and the motor isn’t working. This is a huge safety concern as it could’ve caused a major injury accident, especially if I was on the freeway.
