FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
Potential Consequences:
When using cruise control (with or without auto steering to stay within a lane), the car randomly breaks. This does not appear to be associate with signage along the road. It appears to happen more frequently in the right most lane. There are certain locations where this problem always occurs, but we cannot determine what is different about that particular exit than others. The most dangerous situations have been when the breaking occurs with a car closely following while driving on an interstate in the mountains (I-70). A relative driving my car was almost rear ended at those speeds while driving on I-70. The breaking is random and unexpected, which makes it more dangerous because as driver you are not prepared for this to occur. At a speed limit of 65mph, when you are driving in a curve and suddenly the car brakes, especially when using regenerative breaking, it is the same as slamming on the brakes. This could cause you to loose control over your car. The issue has been known to Tesla but has not been fixed and new cars are advertised and sold with adaptive cruise control when in fact they are faulty, rendering the feature useless at best, dangerous in most cases.

On Saturday, 25 June, while driving our 2022 Tesla Model Y north on highway 395 between Lone Pine and Bishop, California, and while cruise control (but not lane assist) was active, I experienced three instances of so-called phantom braking. More precisely, on three occasions within a 10 to 20 minute window, the car automatically braked sharply despite there being no vehicle or other obstacle ahead of me. Each time, the braking was sudden, forceful, and without justification. Had a car been following closely behind me, I could well have been involved in a collision. I stress that my foot was not on the brake pedal when the phantom braking took place. After the 3rd episode, I turned cruise control off and drove another couple of hours (to Mammoth Lakes) without incident. The following day, my wife and I drove the Tesla to our home in the Bay Area. We did not activate cruise control at any point during this approximately 6 hour drive, and experienced no additional incidents. Understandably, we are reluctant to re-attempt the use of cruise control any time soon. I have owned this Tesla Model Y since January of 2022, and these were the first instances of phantom braking.

EXTERIOR LIGHTING
Potential Consequences:
Turn signal does not always activate. If I hit the turn signal stalk so it goes past the first detent (for three blinks) but not hard enough to get to the second detent (on until cancelled or turn completed), it does not come on at all, either the three blinks or constant blinking. This is dangerous because the turn signal does not activate predictably, especially when changing lanes on the highway. I should be able to tap the turn signal to get the 3 blinks to come on and then switch lanes, but when used this way it does not always come on. I had this inspected by a Tesla Mobile Service technician and he reproduced what I reported but said this is working as intended because it is detecting an "accidental bump" and not activating. My vehicle is available for inspection upon request. There are no warning lights or messages.

Auto dimming and bright headlights do not dim for oncoming traffic consistently. Unsafe for other travelers at night. I’ve owned two Tesla’s model 3 and model y over the last 4 years. Both had issues with the Auto Bright/Auto dimming headlights. The automatic feature takes too long to turn on the high beams and takes too long to dim for oncoming traffic. The auto dimming function also reacts to reflective signs and turns off high beams when unnecessary. The auto dimming feature works best when you’re approaching a curve where the travel path is to the right where oncoming traffic crosses the field of cameras. Auto dimming rarely works when turning to the left or straight for on coming traffic since cars are to the left of field for the cameras or head on traffic but Auto dimming feature fails. The bright and dim will go on and off as cars approach further causing issues with people flashing lights at you in their direction of travel. We park in the center of the garage which is relatively empty with 2 plastic tool boxes on the left side and a Traeger grill on the right side. I am reporting 3 incidences of Phantom braking with adaptive cruise control on, and 2 incidences of phantom vehicle displays on the central console while parking into a garage. 4/17/2022 ~ 1pm. on RT 101 CA between King City and Camp Robert Rest Area/Bradley. Traveling at 75-78 mph on widely separated divided straight highway, with no trees, only low grass, and no cars nearby, we (my wife driving) experienced two sudden jarring braking slowdowns. This is a hazard if there is a car tail-gating us or if the following driver does not expect us to brake for no reason. 4/18/2022 ~ 11am on Rt 10 CA between Palm Desert/Indo and Desert Center, We were traveling at 76 mph on a widely separated divided straight highway, with no trees, and only low grass. A Tesla car in front of us suddenly braked for no apparent reason and shortly thereafter pulled over to the shoulder and stopped. A few minutes later we experienced a sudden braking action in our Tesla car. In May/2022 we observed two separate incidences of a phantom van displayed on the Tesla central console while parking our car into our garage. A phantom van appeared once on the right side and once on the left side when the Tesla car was half-way into the garage. FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
Potential Consequences:
Purchased a Tesla Model Y approx. 3 weeks ago and within the first day experienced sudden, aggressive braking on the freeway with nothing around and no apparent reason for braking at all. This has happened at least ten times in the three weeks we’ve owned the vehicle. It’s very scary when it happens and we’re very concerned it will cause an accident with my family and small child in the back. It happens when the car is in auto pilot and adaptive cruise control. The car gives no alert and no indication that it’s going to brake hard all of a sudden in the middle of the interstate at 70 mph it will instantly drop speed by as much as 15mph or more. Thankfully nobody has been behind us yet. For us this is an extreme safety concern and we have expressed this to Tesla and I have sent out a maintenance technician today who took it for a simple test drive and said there’s absolutely nothing wrong with the car and that’s the safest car in the world and that what we were experiencing is normal behavior for a Tesla and to be expected. He said when it happens to stereo on the accelerator to get it to stop which is ridiculous. This is a known problem generally referred to as phantom braking and I was assured by the sales rep before accepting the car that this doesn’t happen in auto pilot and nothing to be concerned about. Now here we are stuck with a dangerous vehicle that they can’t fix and won’t take it back. Please help get these cars off the road until they fix this huge safety issue. I am shocked there has not been a recall. I will also file a complaint with the state of Washington.
