Defect Description:
LANE DEPARTURE: ASSIST
Potential Consequences:
New update came out and they took some of my advice; however, the problem got worse. I had an appointment at a new dealership 1 hour away and they ask me to see if I can replicate the issue and send them a video of it (yikes this is getting very dangerous) so on my way to the service appointment I do as I'm asked but the car keeps malfunctioning saying "Cheater detected" and I'm driving with my hands on the wheel with my dashcam on and iPhone with camera open ready to just grab it for the picture. I send it to them and try to use FSD again. It malfunctions again, says I'm a cheater twice in five minutes. At this time I'm unaware of what a "cheater" even is other than the traditional term. I get to the dealership and the car pops up with a window says "Full Self Driving has been removed due to constant ignoring of the blah blah blah". I tell the Tesla employee what happened he immediately says there' nothing he can do he cant fix it and I can't get my money back. I got all of this on dashcam and audio fyi. I come back the next day, the second I get out of the car and my foot hits tesla pavement a Tesla employee is harassing me to move my car except its the Uber Tesla paid for me to come pick up my car, side note the guy next to him has a huge handgun on his hip, what a safe feeling welcome... The guy inside tells me what cheaters are, I look it up on YouTube. Basically its a wrist weight people attach to the steering wheel to trick the car. I don' own anything like that the car was confusing it with my apple watch which I told Tesla and was promptly updated to the car in 24 hours. So now I'm driving a car I paid $12,000 for FSD that has driven me into coming traffic many times and calls me a cheater until it steals my $12,000 like some sort of horror arcade game. Now the car only has Auto Pilot which also calls me a cheater so its all un-useable. I have heard every excuse in the book from Tesla. Now they want to start charging me $110 to have the AutoPilot looked at.

STRUCTURE:BODY
Potential Consequences:
As I was going up the ramp from Peachtree-Dunwoody Road onto 285E on Thursday, Jan 5th around 7pm, there is a tree on the left hand side that has long branches that extend into the road. The branches of the tree hit the side of my car. I heard it as I was driving but didn’t think anything of it. When I got home I had the chance to look at my car. There is a scratch on the side of the car that the tree limbs hit. The scratch actually looks like someone keyed my car. My car is brand new and I want to report this incident to prevent others from getting their cars scratched up. It would be helpful if the tree limbs could be cut down to prevent car damage. I also would like to be compensated for the damage to my car. I am happy to provide pics of the tree and my car.

UNKNOWN OR OTHER
Potential Consequences:
There is an outstanding recall 22V702 - automatic window reversal systems and the next course of action is that Tesla will deploy an over-the-air (“OTA”) firmware update to affected vehicles. However, my tesla account doesn't have that software available yet. Registrar of Imported Vehicles has searched the records for the above referenced vehicle and has either detected an outstanding recall, or has been unable to verify the current recall status of this vehicle. So unfortunately, they are unable to release an inspection form until such time as they need confirmation that the recall, if any, has been corrected, or that there are no outstanding recalls for this vehicle. Steps taken by me to eradicate this: 1) I tried contacting the tesla dealer at various locations in Canada and USA. 2) I did reach out to the Tesla manufacturer and they told me that they will try to deploy firmware updated for my vehicle and they told me to wait for 1 business day( this conversation happened like 5 days ago) and some associate from this location will reach out to me and i can visit that service center for the resolution of recall. Tesla dealers along with their service centres are totally unaware of what’s going on and don’t have visibility of when this software will be available. This is very big blocker for me as my car will not be registered in Canada. Can we please resolve this recall so that my import process gets completed. This is surely emergency case where I am unable to drive my vehicle and I will be charged too. I have already exhausted those days. Tesla is unable to solve this. Can we please escalate this issue and solve this recall on my car and also once done Tesla should issue recall clearance certificate that I need to submit to RIV for my inspection process.

FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
Potential Consequences:
Rented the vehicle from Hertz for a holiday road trip of 3100 miles. Experienced multiple instances of "phantom braking" where the cruise control would slam on the brakes with nothing in front of the car within the range of vision. The car would rapidly slow from the posted speed limit to under 45 MPH and then about 5-10 seconds later resume as if nothing had happened. This problem happened to me several times per driving day, and seemed to be more prevalent when cresting a rise in the highway. A couple of time times, this happened with a car behind me, and the sudden braking could have caused the vehicle behind to rear-end the Tesla. In heavy traffic it is easy to see how this phantom braking could cause a chain reaction accident. At this point, I think that Tesla needs to permanently disable their cruise control in all Model Y vehicles.

Defect Description:
SEATS
Potential Consequences:
1. Whoever designed the seats never sat more than 2 hours straight in them and is most likely below 150 lbs with the body shape of a petite person. Damaged coccyx and lower backs since inception of idea. Insanity will ensue after long periods of travel (+2hrs) in rear seating and extended third row seating for adults and children. You get more seating comfort from a Nissan than a Tesla. 2. Windshield wiper sensitivity lowers when using manually then activating Auto. Sometimes sensitivity goes down with autopilot. Auto wiper sensitivity not constant. 3. If you have more than 4 people in your family, do not buy a model S3Y 4. Bad panels which also affect steering column, with squeaking, why is that not a lemon? 5. Long range below 32F falls to half its mileage per full charge. Proper trip charging should be at 10% to 15% more than the minimum 0% for trip charge. 6. Autopilot forces high beams whenever there is fog. Autopilot will never engage on curves, only straight aways 7. Supercharger long trip navigation will not allow filtering locations with bathrooms or trashcans nearby. Walking far distances to find restrooms and trashcans are a normal thing. Teslas not meant for taking long trips. 8. Signage for charging misleading, "10 to 30 minute parking" will only charge 10 to 50 KW max while "vehicle charging only" will charge the advertised supercharger max output. 9. No climate control in third row for MY, safety hazard and human rights issue.

VISIBILITY/WIPER
Potential Consequences:
When I am driving and go into Cruse Control my wipers continue to come on when it is not raining or there is no moisture. Sometimes they are going at full speed as if it is pouring rain. When this happens, I am forced to hit my brakes to disengage Cruse Control, then take my eyes off the road and find the wiper icon and turn them to the off position. This happens often and has almost caused me to drive off the road a few times. I took the car back to Tesla and was told when I engage Cruse Control the wipers are by default set to "Auto". Also they had seen this before and there was nothing they could do to fix the issue. They also said Tesla was working on the software to improve the wipers. Why am I forced to have my wipers set to "Auto" when I use Cruse? I should be able to set them to "Off" and they only come on when I activate them. If this continues on my car and others there will be accidents because of the distraction trying to turn the wipers off.

FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
Potential Consequences:
The contact owns a Tesla 2022 Tesla Model Y. The contact stated while driving at 65 MPH in the rain, the vehicle experienced phantom braking and stopped suddenly. The vehicle then veered to the right independently. The brake pedal was depressed but failed to respond. The contact lost control of the vehicle. The vehicle spun out and crashed into a wall on the side of the road. The air bags deployed. The contact sustained bruises and significant pain to the left leg. The contact also sustained soreness of her fingers, arms and shoulders. The contact did not seek medical assistance. The contact’s sister transported her to her residence. The contact was unsure whether a police report was filed. The vehicle was towed to a tow lot. The vehicle was then towed to a collision center. The contact was not using autopilot mode at the time the failure occurred. The manufacturer was notified of the failure and informed the contact that she would need to repair the vehicle at her own expense. The vehicle was not diagnosed or repaired. The failure mileage was approximately 5,000.

VEHICLE SPEED CONTROL
Potential Consequences:
While driving with Autopilot engaged at approximately 65 mph the car suddenly decelerated to less than 40 mph and then accelerated back to 65. This happened again within a minute of the first occurance and I turned off Autopilot but left cruise control on. It then happened again and I turned cruise control off. Fortunately there were no cars following close behind me nor was I following close behind another car. In the first two cases there were intermittant shadows across the road cast by trees along the road side. In the third there was a large black programmable traffic information sign 20 ft or so off the side of the road that had no information displayed. There was no damage to the car or injuries to the occupants. The vehicle has not been inspected by the manufacturer. No warnings were displayed prior to the event.
Corrective Action:
While driving with Autopilot engaged at approximately 65 mph the car suddenly decelerated to less than 40 mph and then accelerated back to 65. This happened again within a minute of the first occurance and I turned off Autopilot but left cruise control on. It then happened again and I turned cruise control off. Fortunately there were no cars following close behind me nor was I following close behind another car.
