In the event of an unbelted driver, the seat belt warning light and audible chime may not activate as intended. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 208, "Occupant Crash Protection."

Potential Risk:
A seat belt warning system that fails to alert occupants of an unbelted seat belt can increase the risk of injury during a crash.

Recall Date: Jan 30, 2024
Safety Issue:

An incorrect font size is displayed on the instrument panel for the Brake, Park, and Antilock Brake System (ABS) warning lights. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 105, "Hydraulic and Electric Brake Systems" and 135, "Light Vehicle Brake Systems."

Potential Risk:
Warning lights with a smaller font size can make critical safety information on the instrument panel difficult to read, increasing the risk of a crash.

Safety Issue:

Tesla, Inc. (Tesla) is recalling 2012-2023 Model S, 2016-2023 Model X, 2017-2023 Model 3, and 2020-2023 Model Y vehicles equipped with all versions of Autosteer leading up to the version(s) that contains the recall remedy. In certain circumstances when Autosteer is engaged, the prominence and scope of the feature's controls may not be sufficient to prevent driver misuse of the SAE Level 2 advanced driver-assistance feature.

Potential Risk:
In certain circumstances when Autosteer is engaged, and the driver does not maintain responsibility for vehicle operation and is unprepared to intervene as necessary or fails to recognize when Autosteer is canceled or not engaged, there may be an increased risk of a crash.

Recall Date: Feb 15, 2023
Safety Issue:

Tesla, Inc. (Tesla) is recalling certain 2016-2023 Model S, Model X, 2017-2023 Model 3, and 2020-2023 Model Y vehicles equipped with Full Self-Driving Beta (FSD Beta) software or pending installation. The FSD Beta system may allow the vehicle to act unsafe around intersections, such as traveling straight through an intersection while in a turn-only lane, entering a stop sign-controlled intersection without coming to a complete stop, or proceeding into an intersection during a steady yellow traffic signal without due caution. In addition, the system may respond insufficiently to changes in posted speed limits or not adequately account for the driver's adjustment of the vehicle's speed to exceed posted speed limits.

Potential Risk:
FSD Beta software that allows a vehicle to exceed speed limits or travel through intersections in an unlawful or unpredictable manner increases the risk of a crash.
FREE Recall Solution:
Tesla will release an over-the-air (OTA) software update, free of charge. Owner notification letters were mailed April 15, 2023. Owners may contact Tesla customer service at 1-877-798-3752. Tesla's number for this recall is SB-23-00-001.

Safety Issue:

Tesla, Inc. (Tesla) is recalling certain 2016-2022 Model S and Model X, 2017-2022 Model 3, and 2020-2022 Model Y vehicles. The "rolling stop" functionality available as part of the Full Self-Driving (Beta) software may allow the vehicle to travel through an all-way stop intersection without first coming to a stop.

Potential Risk:
Failing to stop at a stop sign can increase the risk of a crash.
FREE Recall Solution:
Tesla will perform an over-the-air (OTA) software update that disables the "rolling stop" functionality, free of charge. Owner notification letters were mailed March 28, 2022. Owners may contact Tesla customer service at 1-877-798-3752. Tesla's number for this recall is SB-22-00-001.

Recall Date: Jan 29, 2021
Safety Issue:

Tesla, Inc. (Tesla) is recalling certain 2012-2018 Tesla Model S and 2016-2018 Model X vehicles with a center display equipped with a NVIDIA Tegra 3 processor and an 8GB eMMC NAND flash memory device. When the 8GB eMMC NAND flash memory device for the center display reaches lifetime wear, the eMMC controller will no longer be able to maintain the integrity of the filesystem, causing a failure in some of the center display functions.

Potential Risk:
The eMMC controller wear-out condition can cause the loss of the rearview camera display, defrost/defog control settings, and exterior turn signal lighting, reducing visibility and increasing the risk of a crash.
FREE Recall Solution:
Owners should ensure their vehicles are operating firmware release 2020.48.48.12 or newer, which will alert owners if the eMMC is approaching lifetime wear. Tesla will notify owners, and will replace the VCM daughterboard with one containing an enhanced eMMC controller, free of charge. The recall began March 29, 2021. Safety Issue:

Tesla, Inc. (Tesla) is recalling certain 2015-2016 Model X vehicles. The aluminum bolts that attach the power steering gear assist motor to the gear housing may corrode and fracture causing a reduction or complete loss of power steering assist.

Potential Risk:
Loss of power steering assist would require a higher steering effort, especially at lower speeds, which may increase the risk of a crash.
FREE Recall Solution:
Tesla will notify owners, and Tesla Service Centers will replace the steering gear mounting bolts with coated steel bolts, and add a corrosion-preventative sealer to the steering gear motor housing and motor connection bolts. If the mounting bolts are found to be broken or break during removal and cannot be removed, a new steering gear will also be installed. All services will be performed free of charge. SERVICE BRAKES

Potential Consequences:

My 2016 Tesla Model X (VIN [XXX] ) was manufactured in March 2016 according to the sticker on the driver door. My car is experiencing the same issue as the Tesla Service Bulletin #SB-17-33-002 under NHTSA's recall # 17V2600 when putting the vehicle into drive or reverse from park. Tesla is stating they can not perform the service work because there is no recall specifically for my VIN# even though the car is showing the same error messages on the diagnostics screen and showing the same symptoms and failure as the recall and service bulletin. The repair requires the replacement of the parking brake calipers which is a safety issue if not replaced which could result in not properly functioning when needed. The vehicle is available for inspection. The vehicle displays warning messages not to drive the vehicle, the parking brake is not released and could cause damage to the brake pads if driven, and to take to the Tesla service center. Tesla is proposing the same repair and solution as the recall / service bulletin and wants to charge me $1,500 for something that should be under the recall, especially since my car was manufactured during the specified timeline of Feb 2016 to Oct 2016 and should fall under the safety issue with the same issue as the recall. This recall needs to include my VIN # and any Model X built during this timeframe. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING

Potential Consequences:

Just got [XXX] yesterday morning. I have ~350 miles on this version now in ~24 hrs. FIRST TIME I've ever completely turned off FSD and went back to Autopilot Only. This release is Absolutely 100% DANGEROUS and needs to be pulled and rolled back ASAP Observations: 1: Does not Honor Turn Only lanes and drives straight through them. 2: CONSTANT lane switching no matter what setting it's on. 3: Lane changes without using turn signal. Example, in right lane, it FULLY changes lane to left lane using turn signal, then decides to just go right back to the right lane again without using turn signal cutting off other vehicles. 4: Speed issues, It can't decide on speeds. 45mph road. I do like 5mph over. With no other cars around, it will speed up to 55-60 (Above max setting) then drop down to 40-43mph. It's all over the [XXX] place! Even worse on Freeway. So it actively exceeds speed limits, as much as enough to get my license automatically revoked under Wisconsin Law! 5: Redesigned where set speed limit is displayed. No longer shows unless you actively change it. Then it disappears again. Need to constantly fiddle with set speed just to see how crazy it will decide to break the law, or how slow it will go and [XXX] other drivers. 6: Rolling stops, had a number of rolling stops today and one where it blew straight through a stop sign. 7: Route choices are absolutely alinidine. Seems to prioritize taking the least efficient routes. 8: Drives like a bucking bronco with accel and decell. 9: Ping pongs within the lane. 10: False emergency braking events. 11: Last release the double stop at stop signs had gotten better. This release they are full back again. It is once again stopping at both sides of the intersection. 12: Vehicle departing lane warnings when driving manually, even when my TURN SIGNAL IS ON so I can pull into a parking lot or my own driveway! With the above, there have been no other changes other than firmware update. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C.

SERVICE BRAKES

Potential Consequences:

To Whom It May Concern, My name is [XXX] , and I am writing regarding unresolved issues with my 2016 Tesla Model X (VIN: [XXX] ). Despite multiple service visits, extensive repairs, and approximately $8,000 in costs, my vehicle remains undrivable and presents significant safety concerns. I initially brought my vehicle to the Tesla Service Center in Richmond, Texas, for motor-related issues. I was informed that mismatched tires and front-end components were contributing to the problem, and repairs were completed. However, the original issue persisted. Subsequent visits for additional repairs also failed to resolve the problem, and new safety concerns emerged. Complicating matters, the Richmond Service Center has since closed, and its staff were terminated. When I contacted the Westchase Service Center, Manager Andrea P. informed me that no resolution or agreement could be reached. I have attached the following invoices for your review: Invoice #[XXX], dated April 26, 2024 Invoice #[XXX], dated November 9, 2023 I am requesting the following: A full review of my service history and unresolved issues. Immediate repairs to address motor and safety concerns at no additional cost. A clear explanation of Tesla’s policies regarding unresolved repairs, especially with closed service centers. This situation has caused financial and personal hardship. I urge Tesla to address these issues promptly to restore my confidence in your brand. Please contact me at [Your Phone Number] or [Your Email Address] to discuss this matter. Corrective Action:

POWER TRAIN

Potential Consequences:

While driving on city streets at very low speeds, error messages starting popping up on the screen "Vehicle may not restart/Service is required". We somehow made it home by restarting the computer and driving home at low speeds. At freeway speeds, this would have been catastrophic. We were able to make an appointment with the Tesla service center and found this to be due to the battery coolant heater to be internally shorted which in turn was because the large rear drive unit had an internal coolant leak. This required a very expensive $8k replacement of the large rear drive unit and the heater. A search in the blogs has yielded this to be a design and manufacturing issue. Specifically the blogs say: "the rear motor used in the early Teslas has a design flaw and will need a very expensive repair/replacement but what does that actually mean? Many Model S and Model X produced between 2012-2020 have the Large Drive Unit (LDU) with the fatal coolant seal flaw where the seal around the rotor shaft will leak coolant into the motor (Reference 1). They will ALL leak eventually. Tesla tried many things over the years to keep this seal from leaking but finally gave up and developed ‘Revision U’ in the fall of 2022 which deletes the coolant from the rotor but maintains the coolant to the inverter, stator and the ‘flyover’ tube cooling the gearbox". This means that the same problem even after the replacement, will happen again soon. I am writing to request you to send out a recall to all owners of these cars, and to have Tesla refund us for the cost of replacement of the drive unit and for Tesla to install the bypass as some aftermarket vendors (Reference 2) have started doing.
