Potential Risk:
An inoperative horn may prevent the driver from alerting other drivers, increasing the risk of a crash.

Tesla, Inc. (Tesla) is recalling certain 2021-2025 Model S and Model X vehicles. The driver's air bag could tear during deployment. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) numbers 208, "Occupant Crash Protection" and 212, "Windshield Mounting."

Potential Risk:
A torn air bag may not adequately protect an occupant in a crash, increasing the risk of injury.

In the event of an unbelted driver, the seat belt warning light and audible chime may not activate as intended. A factory reset muted the Pedestrian Warning System (PWS) sounds. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 141, "Minimum Sound Requirements for Hybrid and Electric Vehicles."

Pedestrians may be unaware of an approaching vehicle if the PWS sounds are muted, increasing the risk of a crash.

An incorrect font size is displayed on the instrument panel for the Brake, Park, and Antilock Brake System (ABS) warning lights. In certain circumstances when Autosteer is engaged, the prominence and scope of the feature's controls may not be sufficient to prevent driver misuse of the SAE Level 2 advanced driver-assistance feature.

In certain circumstances when Autosteer is engaged, and the driver does not maintain responsibility for vehicle operation and is unprepared to intervene as necessary or fails to recognize when Autosteer is canceled or not engaged, there may be an increased risk of a crash.

Tesla, Inc. (Tesla) is recalling certain 2016-2023 Model S, Model X, 2017-2023 Model 3, and 2020-2023 Model Y vehicles equipped with Full Self-Driving Beta (FSD Beta) software or pending installation. The FSD Beta system may allow the vehicle to act unsafe around intersections, such as traveling straight through an intersection while in a turn-only lane, entering a stop sign-controlled intersection without coming to a complete stop, or proceeding into an intersection during a steady yellow traffic signal without due caution. In addition, the system may respond insufficiently to changes in posted speed limits or not adequately account for the driver's adjustment of the vehicle's speed to exceed posted speed limits.

Potential Risk:
FSD Beta software that allows a vehicle to exceed speed limits or travel through intersections in an unlawful or unpredictable manner increases the risk of a crash.

The driver side exterior rearview mirrors are convex (curved glass) and the passenger side mirrors do not include the "Objects in Mirror Are Closer Than They Appear" etched warning. As such, these mirrors fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 111, "Rear Visibility."

Potential Risk:
A lagging or restarting CPU may prevent the center screen from displaying the rearview camera image, gear selection, windshield visibility control settings, and warning lights, increasing the risk of a crash.

The Boombox function allows sounds to be played through an external speaker while the vehicle is in motion, which may obscure the Pedestrian Warning System (PWS) sounds. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 141, "Minimum Sound Requirements for Hybrid and Electric Vehicles."

Potential Risk:

Company owner clearly stated that a center airbag enabled horn would be available on all cars produced from November 2021 forward and would be activated by a firmware update. Either the hardware was never deployed or the update was never activated because no one has received it. Tesla will not confirm or deny that the newer production cars even have the necessary hardware to be updated. Find it shocking that this has not been addressed from a regulatory standpoint by now. Company should be mandated to deliver what their owner has promised after having released a new model that is lacking a practical, safe version of a significant safety feature.

I have had this car for about three months. It is equipped with "yoke" steering. I have fully adapted to it, with the exception of the horn button. Because I use it so infrequently, I have had no chance to develop muscle memory, and I suspect I am not alone. Twice now, while in heavy fast traffic in a middle lane, I've been in the blind spot of a car to my right. That car moved at a reasonable pace into my lane, and I could have stopped the movement with a rapid warning honk. Because I couldn't locate the horn button quickly, I was forced to brake abruptly and swerve left. Fortunately, both times the drivers to my left saw what was happening and moved left as well, but it could have been much worse. I am terrified of a child running toward the street and I can't honk fast enough. I keep practicing the horn, but I have 50 years of middle wheel push to overcome. Much as I love my Plaid, I think they should re-think this critical safety issue.

Only once was there another car in our vicinity and that car was next to us within their lane passing us at the time. All 4 times the cars collision warning chimed and hard emergency braking was applied by the car for no reason. If there had been cars following us we would have been rear ended. To date these are the only times this has happened. Tesla says they have no current fix. There was no indication of any problems with the auto pilot/collision warning prior to this.

experiencing phantom braking. On a recent trip I had several episodes of phantom braking where the vehicle slammed on the brakes without any obstacles in front of the car. Just an open road. While this has happened on two lane roads with oncoming traffic on this recent trip it happened on a freeway with nothing in front of it.
