The FSD Beta system may allow the vehicle to act unsafe around intersections, such as traveling straight through an intersection while in a turn-only lane, entering a stop sign-controlled intersection without coming to a complete stop, or proceeding into an intersection during a steady yellow traffic signal without due caution. In addition, the system may respond insufficiently to changes in posted speed limits or not adequately account for the driver's adjustment of the vehicle's speed to exceed posted speed limits.

Potential Consequences:
The contact owns a 2018 Tesla Model 3. The contact stated that upon reversing out of a parking lot, an abnormal clunking sound was coming from the vehicle while turning to the right. There was no warning light illuminated. The contact continued driving and the steering wheel became loose, and an unknown object detached from the vehicle upon entering her driveway. Due to the failure, the contact decided to have the vehicle towed; upon arrival, the tow truck driver discovered that a bolt had fallen out of the vehicle. Once the vehicle arrived at the service center, the mechanic replaced a lower front lateral link and half shaft assembly. Upon investigation, the contact discovered NHTSA Campaign Number: 23V235000 (Suspension); which she linked to the failure. The manufacturer was not notified of the failure. The vehicle was repaired. The failure mileage was 70,950.

(Please note the clock time on the vehicle is incorrect—it is about 5 minutes slower and sets to a time I hr. earlier than the current eastern time). Vehicle: 2018 Model 3 VIN [XXX] The computer system failed in the middle of highway. The computer screen where all system functionality is monitored went bank except for a Tesla icon. Losing control of vehicle including turn signals in the middle of the highway was dangerous and a safety concern. For fear of being stranded on the road I carefully drove the vehicle to home in Yorktown Va. It was scary! Much of the settings on computer and navigation info was lost. This is a vehicle defect I need to report. INFORMATION Redacted PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6).

I received a message on the cars screen Steering alignment service recommended On the drivers side 2 bolts fell out at low speed which we picked up behind the vehicle. Using a jack one of the bolts was reinstalled to allow it to be driven onto a tow truck. I researched and found you had a recall on 2018 model 3 for bolts coming loose. It appears my vin was not part of this recall . I feel it should have been obviously as they must have worked themselves loose to fall out. I discussed this with Tesla service and they are quoting me $2500 for repairs because my vin was not included in the recall.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
Potential Consequences:
This is an issue regarding the full self driving software. My car continues to run red lights at which it is already stopped. The light is solid red to the naked eye, and the camera definitely does not show the normal steady blink of a malfunctioning signal. The pitch forward in the video is me slamming on the brakes as the car tries to run the light (about 28 seconds in). I have previously reported this to Tesla on 12/28/21, 8/25/22, 9/21/22, 11/21/22, and 4/24/23. It has happened many more times than that, as I usually don't see a point in reporting the same bug multiple times on the same software version. As far as I know, nothing is being done about this and I have received no acknowledgement of the problem. The fsdbeta@tesla.com autoreply states that they forward this information to the correct department. This is, in my opinion, many orders of magnitude more serious than the previous Tesla recall regarding waiting too long at stop signs as this will cause a crash for an inattentive driver. https://youtu.be/ZvJASdlEEPU

This is an issue regarding the full self driving software. My car continues to run red lights at which it is already stopped. The light is solid red to the naked eye, and the camera definitely does not show the normal steady blink of a malfunctioning signal. The pitch forward in the video is me slamming on the brakes as the car tries to run the light (about 28 seconds in). I have previously reported this to Tesla on 12/28/21, 8/25/22, 9/21/22, 11/21/22, and 4/24/23. It has happened many more times than that, as I usually don't see a point in reporting the same bug multiple times on the same software version. I was experiencing intermittent Bluetooth connection issues along with vehicle not powering off and the gear shifting to park when moving the vehicle at (very) low speed without the seatbelt buckled. I noticed the Bluetooth connection problem after a bad software installation in Jan 2023, however the other two issues have been noticed since at least 2020. When I took the car for a second time, Tesla diagnosed the problem as a faulty seat occupancy sensor (SOS). Tesla told me that the driver side seat SOS is “sticky” that the car didn’t consistently know there was a driver in the car when I sit on the seats (which explained why the car would put itself in park when driven at low speed (<3mph) without seatbelt on, and why the engine kept running even after I stepped out of the car when the car wasn’t locked ( like, when the car is parked in the house’s garage). Further research on the web (https://www.carproblemzoo.com/tesla/model3/2018/seats-problems.php ) shows that other teslas had faulty seat occupancy sensors at both driver side and passenger side. Faulty sensors may lead to improper, or failed, airbag deployment. This is a safety issue that Tesla had known and should have performed a recall and fixed, or the very least sent a billeting to its service centers so that they would check the seat sensor when someone’s complaining about any of the three symptoms I had. When I took the car to Tesla in Feb 2023 their (mis)diagnosis was that I had too many wires plugged into USB ports and that’s why the Bluetooth didn’t connect. I didn’t complain about the vehicle going to park by itself - as I thought it was a “safety feature”. I didn’t care to report the motor running after exiting, as I thought it was the battery mgmt system running to cool the battery!

I noticed the Bluetooth connection problem after a bad software installation in Jan 2023, however the other two issues have been noticed since at least 2020. When I took the car for a second time, Tesla diagnosed the problem as a faulty seat occupancy sensor (SOS). Tesla told me that the driver side seat SOS is “sticky” that the car didn’t consistently know there was a driver in the car when I sit on the seats (which explained why the car would put itself in park when driven at low speed (<3mph) without seatbelt on, and why the engine kept running even after I stepped out of the car when the car wasn’t locked ( like, when the car is parked in the house’s garage). Further research on the web (https://www.carproblemzoo.com/tesla/model3/2018/seats-problems.php ) shows that other teslas had faulty seat occupancy sensors at both driver side and passenger side. Faulty sensors may lead to improper, or failed, airbag deployment. This is a safety issue that Tesla had known and should have performed a recall and fixed, or the very least sent a billeting to its service centers so that they would check the seat sensor when someone’s complaining about any of the three symptoms I had. When I took the car to Tesla in Feb 2023 their (mis)diagnosis was that I had too many wires plugged into USB ports and that’s why the Bluetooth didn’t connect. I didn’t complain about the vehicle going to park by itself - as I thought it was a “safety feature”. I didn’t care to report the motor running after exiting, as I thought it was the battery mgmt system running to cool the battery!
