Safety Issue:

Maintainer Corporation of Iowa, Inc. (Maintainer) is recalling certain modified 2019-2020 International CV 515 and Chevrolet Silverado tow vehicles equipped with Allison 2700 series transmissions with Chelsea Power Take Offs (PTO). The PTOs were installed incorrectly and as a result, the attachment hardware may loosen over time.

Potential Risk:
If the PTO attachment hardware loosens it could cause a loss of transmission fluid which may result in the rear wheel locking up, increasing the risk of a crash.
FREE Recall Solution:
Maintainer will notify owners, and dealers will inspect the PTO and install a new attachment stud kit, free of charge. The recall began June 9, 2020. Owners may contact Maintainer customer service at 1-800-831-8588.

Defect Description:

SERVICE BRAKES, HYDRAULIC

Potential Consequences:

The contact's company owns a 2020 Chevrolet Silverado. The contact stated that a driver was driving the company vehicle, and as he approached a traffic signal, he reported that the brakes had failed and the speed was unknown. The contact stated that the driver managed to turn the vehicle to the right off of the roadway to avoid cross traffic and went down a slight embankment. The front end of the vehicle crashed into the bottom of the embankment. The front axle was sheared from the vehicle, the hood was crushed. The contact stated that the vehicle was towing an enclosed trailer that had tipped over. The contact stated that including the driver there were six total people in the vehicle. The driver's leg had been pinned by a damaged brake and accelerator pedals, and the driver's left leg had been broken. The driver also had cuts and abrasions to his face and arms and had pain in his chest. The air bags in the front and the side air bags had deployed. Passenger 2 had abrasions to his head and face and had injured his mouth. Passenger 2 also had rib pain on the left side, left knee pain, and a fractured left leg. Passenger 3 had sprained a shoulder and arm (unknown arm). Passenger 4 had back pain and no other injuries. Passenger 5 had suffered abrasions and cuts to arms and bruising to arms and legs, and refused to be transported by ambulance to the emergency room, however, he did seek medical attention the following day. The other 5, driver included, were transported by ambulance to the emergency room. A second vehicle which had been in the cross traffic had had its rear bumper sheared off by the contact's vehicle's left driver's side. The contact stated that only two of the three people in the vehicle were taken by ambulance to the emergency room, and injuries were not reported. The police were on the scene and filed a report.
